school to undergo Work Immersion at

his/her Work Immersion subject. I am giving consent with my full

knowledge that the Work Immersion is held in relation to the chosen

track with the following objectives: to give an exposure and learning

experiences; to develop knowledge and skills by providing him/her with

opportunities to become familiar with the work place; and to apply his/her

knowledge and skills in the field of specialization

I shall not hold the school, Talon-Talon Senior High

School,/Principal/Coordinators/Supervising Teacher/Partner Agency

responsible for the untoward incident that may happen beyond his/her

control during the duration of the Work Immersion..
