APPOINTMENT LETTER

Dear Ma’am / Sir,

Good day! We, 2nd year college students of the Bachelor of Science in Agriculture major

in Crop Science, studying at Davao Oriental State University – Banaybanay Extension Campus,

are seeking permission from your good office for us to conduct a baseline survey in your

barangay as part of our activity. Hence, this activity will be free of charge, and rest assured that

this activity will alleviate the status of the barangay.

This baseline survey is to know the current situation of your community. A baseline

survey contains some basic information about the community or organization. This would tell us

a lot about the community that we are going to deal with. A baseline study is basically a

gathering of data about the present condition in the community we want to work with before our

project starts. Somehow, a baseline survey allows us to outline where we are and where we are

going, in terms of the extension project that we are going to implement in the area. To simplify,

it measures the key indicators before a project even begins in contradiction of which change and

progress can be measured. This could later be the basis for the impact assessment later on.
