Please include the following items when sending in your warranty claim:
1. This printed form showing your Warranty ID number (142019)
2. A valid proof of purchase/receipt clearly showing the place of purchase AND the
date of purchase
Failure to include a receipt (or valid V-MODA.com order number) will
result in your warranty not being processed and mailed back to you
as is.
3. Your product/headphones (in a case preferably)

Once your warranty package is received by us to the above address, you will be
notified via email and we will begin the repair process (which can take between 1-2
weeks).
If you're an international customer and do not wish to mail your headphones in, you
may contact our warranty department at support@v-moda.com for further options.
