Presenting complaints:

This patient is suffering with disease vesicle stone or bladder stone a few days

Initial observation

He is11 year old child .the physical appearance is weak. The color of his face pale due to
the severe pain .the pulse rate is high.

History of present problem

The patient is suffering with bladder stone last few days .a few days back, its urine
stopped suddenly. Ultra sound found a small piece of stone present in urine bladder. This is the
common disease which can happen any one and any stage of life .according to his family history
there are two or more member of his family victimized this disease.

Psychological Assessment

His psychological assessment of HFD shows that poor coordination, Intellectual


Inadequacy anxiety and delusion of grandiosity.

He scored 107 in self-esteem. His self-esteem assessment shows that average self-esteem.

Behavior observation:

Her facial expression is ups and down during interview due to pain .the patient was very
cooperative. Sometime patient behavior irritable.

Case formulation

The patient was suffering with bladder stone. This disease may affect their behavior and
he shows aggression toward other and have a low self-esteem and may also affect their
socialization .the patient had bio psychological and medical problem.

Management and planning

Counseling:

His counseling is needed to be done .CBT can be used for his behavior modification.

Presenting complaints:

This patient is suffering with chicken pox and fever. High temperature (fever), aches and
headache often start a day or so before a rash appears. Spots (rash). Spots appear in crops. They
develop into small blisters and are itchy. They can be anywhere on the body. Several crops may
develop over several days. Loss of appetite or feeding problems.

Initial observation

The physical appearance is weak. The color of his face red due to the rashes .the pulse
rate is high. The patient is very depressing. According to the initial reading fever is high grade.

History of present problem

The patient is suffering with chicken pox last few days a few days back. Chickenpox can
be serious, especially in babies, adolescents, adults, pregnant women, and people with a
weakened immune system. This is the common disease which can happen any one and any stage
of life .according to his family history there mother was also suffered with chicken pox many
years ago.

Psychological Assessment

His psychological assessment of HFD shows that poor coordination, Intellectual

He scored 88 in self-esteem. His self-esteem assessment shows that approximately average.
