This document will be developed into a more comprehensive Site Safety Management Plan (SSMP) after award of the contract. The SSMP will be implemented in all elements of the scope of the works by Shimizu Corporation in undertaking the Capacity Enhancement of Mass Transit Systems in Metro Manila, LRT Line 1 - South (Cavite) Extension, Expansion of the Existing Depot at Baclaran and Construction of a New Satellite Depot at Zapote. All staffs and personnel, including subcontractors, will comply with the requirements of the SSMP. This document is presented in line with ISO 9001. 2.0 SAFETY MANAGEMENT SYSTEM Each part of Shimizu Corporation is registered for meeting the requirements of OHSAS 18001 and represents the commencement point for the future development of the SSMP for the project. ‘Shimizu Corporation will maintain and implement the SSMP and assure that the system remains in effect during the execution of the works. The safety activities will be planned, performed and recorded to ensure that all work is performed in accordance with the requirements of the contract. Shimizu Corporation will continuously review and update the system to meet the requirements and development of the works throughout the duration of the Contract to the satisfaction of Department of Transportation (DOT?) The OHSAS 18001 certificates for the safety system of Shimizu Corporation are incorporated as Appendix 4 2.1 Safety Objectives and Targets Shimizu Corporation's safety objectives and targets are as noted hereunder: (a) Provide safe working practices for carrying out normal operations of the worksite according to OHSAS 18001. (b) Provide identification of hazards and establish safeguards against such hazards. (c) Provide for response to emergency situations. (d) Strive to continuously improve safety management skills. (e) Create awareness that safety is an integral part of the Works and that production and safely are inseparable. (f) Provide thorough investigations into safety or near miss incidents and establish safeguard to prevent recurrence. (g) Identify lines of responsibility for safety. (h) Provide a safe place of work. (i) Describe how health and safety will be assured throughout the life cycle of the project. (k) Demonstrate commitment to the highest possible health and safety standards. 2.2 Updating Site Safety Management Plan The SSMP will be developed and will be submitted to the Engineer for review and approval within 42 days after the date of commencement or 7 days prior to the start of significant works, including temporary works, on site, whichever is sooner. The SSMP is a live document that will be regularly reviewed and updated as required in accordance with the activities and experiences at the site, KPI (Key Performance Indicators) will help to identify trends of the performances and the creation of action plans which in turn will help deliver improvements to the Site Safety Management Plan. Such development will enhance the standards of safety being implemented at the site. In addition, Shimizu Corporation will review the document implementation at frequent intervals, considering all matters pertaining to safety planning and implementation, including accident reports, inspections, audits, suggestions from meetings and other sources, including the Engineer and its insurance representative, and hazard analysis reviews. 2.3 Laws and Regulations ‘Shimizu Corporation will ensure that operations executed under the Contract will, at all times, be carried out to meet all requirements pertaining to health and safety prescribed in the currently applicable laws in the Philippines 3.0 SAFETY POLICY 3.1 Safety Policy Statement A safety policy statement, declaring the assurance of each member of Shimizu Corporation to the site safety will be submitted to the Engineer. Shimizu Corporation's senior management has formulated a policy which comprises six principle elements. These principles have been developed for use by Shimizu Corporation senior management in order to lead the organization towards improved performance. Employees at all levels of the organization are expected to fulfil the requirements of the policy in all of their work-related efforts and decisions. The Shimizu Corporation safety policy statement is incorporated as Appendix 2. This safety policy statement proposal will be reviewed, updated and officially signed during the development of the SSMP. Itis the responsibilty of the Health and Safety Officer (HSO) to ensure that the policy is effective and carried out in accordance to the applicable government and client standards; it must be well circulated, displayed at the site office notice boards and translated into languages understood by the various nationalities working on site. Revision: 3 oO Site Safety Management Plan Date: 19 Sept. 2019 sHmz Page 7 of 31 3.2 Safety Organization The safety organization will comprise a site safety committee led by the Health and Safety Officer (HSO) and supported by a safety staff and other project staff and subcontractors. The project manager (PM) will be responsible for updating the safety organization chart whenever new safety or site personnel are posted to the job site. The HSO, who will be appointed within 30 days of the date of Commencement of Works, will be solely connected with safety aspects of the works (including environmental, industrial health and hygiene) providing advice and guidance and will be directly responsible and accountable to the PM for all site safety and health matters. He will implement, maintain and monitor all aspects of safety in compliance with the SSMP and all safety procedures. ‘A supporting team for the HSO will be provided to ensure the safely procedures are properly executed at site 3.3 Management Participation in Safety To demonstrate the management's commitment to safety, the PM, engineers and supervisors of Shimizu Corporation will inspect the site regularly, participate in the safety patrol and will properly and fully record the results of the inspections conducted 3.4 Responsibilities and Duties of Safety Personnel Details of the responsibilities and duties of the safety personnel are incorporated as Appendix 3. 4.0 SAFETY WORK PRACTICES. 4.1 General Safe Work Practices The objective of the safe work practices is to eliminate or reduce to a minimum the risk of injury and fatality to persons and damage to assets and properties during the execution of construction activities. All Works are to be carried out in accordance with the SSMP and statutory requirements and in accordance with work procedures, method statements, risk assessments and permit to work systems relevant and applicable to the Work, When planning for work assignments, Shimizu Corporation and the Subcontractor's supervisors must inform the crew or group involved on the safety practices, work methods and personal protective equipment required. Shimizu Corporation and its ‘Subcontractors will be responsible for providing each worker with the proper protective equipment and suitable tools for the work assignment. Work procedures and instructions relating to safety will be written, reviewed periodically and records maintained. Revision: 3 om Site Safety Management Plan Date: 19 Sept. 2019, sHmZ Page 8 of 31 The PM together with the HSO and his subordinates will conduct a survey of the surrounding site before commencement of the project, to reduce the chances of accidents occurring which could pose a danger to the public and the neighboring environment. The PM, assisted by the HSO, will ensure that the following general work practices are being implemented: (a) Good housekeeping practice: (i) Designated areas will be allocated for materials and waste storage. (ii) All materials will be properly stacked and placed in such a manner that it will not affect the safe movement of people and equipment. Good housekeeping will be enforced to ensure a safe and tidy work place (iil) At the end of each working day respective site supervisors and subcontractors are to ensure that all workers working in their areas of work clear any debris, and stack the unused materials properly. (iv) The HSO and the safety supervisors at site will check and confirm that the housekeeping is done properly. (v) Rubbish bins will be provided at designated and visible locations to allow workers to properly dispose and segregate their rubbish, (vi) Protruding nails will be removed or bent over to prevent people from injury. (b) Disposal of Debris: (i) Where appropriate, rubbish chutes of sound construction will be used to dispose of construction debris from the upper levels under construction. (li) The reckless throwing of debris outside the area will be strictly prohibited, (ii) All debris accumulated at site especially on working platforms will be cleared on a regular basis. (iv) Suitable receptacles for wastes, scraps, etc. will be provided, (v) Combustible wastes, such as oily rags, paper, wood, ete, will be stored in a safe place before disposal from the site. (c) Personal Protective Equipment (PPE) (i) Safety helmets of the type approved by an acceptable authority will be provided and worn by all the workers and staff while working at site including authorized visitors, ) Safety belts/harnesses of the approved type will be provided to workers to work in or at areas where they are liable to fall from height. am Site Safety Management Plan Date: 10 Sept 2010 sama Page Be 3 (iil) Safety spectacles, gloves and earplugs will be provided to workers working at work sites that require necessity. (iv) Records of the issuance of the PPE will be documented and kept by the HSO. (d) Safe Place of Work: (i) Appropriate barricades must be provided in the following areas: * All construction work areas’ borders to separate the construction and public area. Unauthorized entry to the site will be forbidden. ‘+ Allopen sides of any uncompleted structures. * Hard barricades will be kept within the floor openings and deep excavations, (ii) All workers working at height must be provided with a safe means of access, secured working platforms, hand rails, ladders, a safety harness with lifeline & must undergo a work at height training. (iii) All electrical installation will be carried out by a qualified and certified electricians. Electrical equipment will be of good construction, sound materials free from defects and maintained according to the manufacturer's instructions. (iv) All flammable and combustible materials will be used with precaution. The storekeeper will be responsible for ensuring that all flammable and combustible materials are stored, used and disposed in a safe and appropriate manner. (e) Machinery Safety: (i) Safety staff will check / inspect all equipment, such as air receivers, lifting equipment, hoists, cranes etc, at regular intervals in accordance with the statutory requirements. Copies of the inspection reports will be retained in the Shimizu Corporation site office for record purposes. (ii) Only trained, competent and qualified persons will be appointed as machinery operators. Unauthorized worker(s) will not be permitted to operate any machinery at site. ) i) All moving parts of any machine/equipment will be guarded as appropriate, (iv) Before any work or maintenance is performed on any machinery, equipment, tool or electrical system, they will be made totally safe before work starts by removing any source of energy or power to it, such as electrical, airlhydraulic pressure, spring/ stored energy or thermal (heat/oold) (v) The Lockout/Tag-out provides a safe method of working on, near, or in machinery or equipment that can cause serious injury. This system will be used by all employees to ensure that the machine or equipment is stopped, isolated Revision: 3 oO Site Safety Management Plan Date: 19 Sept. 2018 Amz Page 10 0f 31 from all potentially hazardous energy / electrical sources and locked out before employees perform any servicing or maintenance where the unexpected energization or start-up of the machine or equipment or release of stored ‘energy could happen and cause an injury. (vi) When the machine guards are removed for maintenance purposes, warning signs will be posted at the machine to warn other workers from approaching or using the machine (g) Use of Tools: (i) Alltools on site must be of sound state and in good working condition (ji) When not in use, tools will be stored in suitable boxes or containers or hung on racks. No tools are to be placed near the edge of the structures where they can fall and pose a falling object hazard (ii) All damaged or worn out tools will be repaired immediately or disposed of appropriately. (iv) Power hand tools are required to be maintained. Unguarded tools should be tagged or out of service until the guard is replaced. (v) Tools should only be used for their intended purpose. No improvised usage will be tolerated. (vi) Tools will be provided with grounding connections and will be insulated, (h) First Aiders and First Aid Facilities: (i) The PM will arrange for an appropriate number of first-aid facilities at site in accordance with the local regulations. (ii) First shall only be given by a First Aider or person trained in First Aid. i) A Supervisor will complete an Accident Investigation Report as soon as possible after an accident. (iv) All minor injuries will receive First Aid to prevent them developing into serious fections, (v) A supervisor or manager shall make the decision whether outside medical treatment is required. (vi) A supervisor or manager must be notified immediately in case of accident. Revision: 3 om Site Safety Management Plan Date: 19 Sept. 2019 ssHmz Page 11 of 34 4.2 Legal Requirements on Safe Work Practices A\list of all the legal requirements on safe work practices will be included in the SSMP. The PM is responsible for checking which local laws and regulations are applicable and ensuring that all legal obligations arising from these regulations are met. In addition, during the construction period up to the commissioning stage of the project the new RA. 11058 (S.Aug/17/2018) coupled with DOLE-DO no, 198-IRR (S.06/Dec/2018) will be complied. 4.3 Identification of Task Requiring Safe Work Procedures A safe work procedure is a formatted written procedure describing how the works will be undertaken in a safe manner. A safe work procedure will be developed for all high- risk activities or where a large volume of hours is expended on a specific task. The PM will decide the type of work activities that require safe work procedures. This will be applied to the following works: (a) Work on any machinery where fencing has been removed for the purpose of any examination, lubrication or other operation, (b) Work at a place where a person is liable to fall a distance of more than 3m or into any substance that is likely to cause drowning, poisoning, and/or chemical burns or asphyxiation. (0) Work in any confined space. (d) Work involving the application of heat, or potential generating of any source of ignition, or where any explosive or flammable substance is liable to be present. (e) Maintenance or repair work on any pressure vessel or lifting equipment. (f) Work on any process, plant, vessel or machinery that is liable to produce or give off any corrosive, toxic or flammable substance. (a) Work in compressed air environment or under water. (h) Work on deep excavation. (i) Work on crane operations involving lifting activities. 4.4 Documentation The HSO will review the general and specific safe work practices, whether produced internally or provided by subcontractors. He will ensure the proper documentation of all general or specific safe work practices. The HSO will also ensure that copy(ies) of the general and specific safety practices are available at the site office for easy reference by any interested parties. He will ensure that for specific work practices, the relevant engineer, supervisor or subcontractor must be issued with a copy of the safe work practice(s). Revision: 3 om Site Safety Management Plan Date: 19 Sept. 2018 sHmz Poge 1 f31 The PM will ensure that all general safe work practices are in place immediately before the works commence and that all the specific work practices are made available; 4.5 Communication The HSO will ensure that the general and safe work practices are communicated to the relevant site personnel. This may be done during: {a) Site meetings. (b) Site safety plan committee meetings. (c) Toolbox meetings. (d) Mass safety talks. (e) Safety Inductions. (f) Safety Patrol. (g) Any other meetings. The HSO will ensure that such communication of safe work practices is properly documented so that records are available for safety audits and review. The HSO will ensure that all site personnel are briefed at least once on all relevant safe work practices while working on site. Communication facilities, to enable the HSO and Shimizu Corporation's other key staffs to communicate efficiently especially on safety matters, will be provided. 4.6 Implementation It is the responsibility of every personnel member working on site to be familiar with and comply with the safe work practices while working on site, The HSO or the safety supervisors at site will report to the PM on any contravention to the stated safe work practices of Shimizu Corporation. The PM will immediately act on any contravention reported by the HSO or any safety personnel. 4.7 Method Statements twill be the duty of the PM and the HSO to review and approve all the work method statements, particularly in terms of safety. ‘Subcontractors undertaking works will ensure their supervisor in direct contro! of the job, briefs and discusses the following with the workers before work commences: (a) Nature of the job. (b) Associated hazards. (©) Safe work method to be adopted 4.8 Permit — To - Work System A permit-to-work (PTW) system provides a documented system of control and notification of a contractor's activities during the stipulated time duration. The HSO will ensure that a Permit-To-Work will be prepared and approved before a specific task or work will be initiated. This generally applies to considerably hazardous activities, Revision 3 (2) Site Safety Management Plan Date: 19 Sent. 2018 samz Page 1331 5.0 SAFETY TRAINING 5.1 Purpose To equip Shimizu Corporation's personnel at all levels to be competent with all the necessary safely knowledge, skills and the correct altitude towards safety compliance and safety consciousness. This will enable the management to control the safety at site and to enable workers to perform their duties in a safe manner to minimize the risk of hazards. 5.2 Internal Training 5.2.1 Safety Induction Course All new personnel of Shimizu Corporation or the subcontractors will on the first day at site before commencing their respective works, attend a Shimizu Corporation in-house safety induction course on the basic safety requirements of the project and the significant features of the construction work relating to safety. It will be the responsibility of the PM to ensure that all personnel of Shimizu Corporation that are entitled to work on the site, including those from subcontractors, are given a safety induction training on their first day of work in the project and it will be used as a pre-condition for the issuance of a security pass to the respective personnel. Thus, any personne! without a valid security ass will not be allowed to enter the worksite. It will be the responsibility of the HSO to conduct the in-house sefety inductions for all personnel entitled to work on site and to maintain records of these in-house safely inductions at the site office. The primary objectives of the induction course are to: (a) Stress the importance of safety and the safety policy. (b) Teach how to avoid accidents. (c) Contribute in developing project safety. (d) Guide the personnel in practicing safe working condition(s). (e) Stress the importance to meet the safety target. (f) Stress the importance of good housekeeping. (g) Inform personnel on the emergency evacuation procedure. (h) Highlight main hazard(s)/risk(s) involved in the works where they will be engaged, (i) Explain the safety rules and regulations that are implemented on the project. () To communicate the hazard control information, Om Site Safety Management Plan Dek 2 ane same Page 14 of 31 Allattendees of the safety induction course will be required to sign an undertaking agreement letter to comply with all relevant acts and regulations, in-house safety rules and regulations and other safety requirements. 5.2.2 In-House Safety Training It is the responsibility of the HSO to conduct internal safety training for all site staff, subcontractors, operators and workers as and when necessary. An in- house safety training schedule will be prepared that complements the type of construction activities to be carried out at the site. The in-house safety training will then be carried out to equip all site staff and workers with the necessary safety knowledge to carry out the planned activities safely and efficiently. In-house safety training may be provided in the following areas; (a) On the job instruction” briefing (b) Safe work procedures (c) Toolbox meeting/ training (d) Safety supervision and monitoring (e) Safety related information distribution/ updates (f) Safety handbook/ leaflets issuance (g) Safety memos or direction updates (h) Work at height training 5.2.3 Safety Trai 1g Records {twill be the responsibility of the HSO to maintain all safety training records of all the site staff, subcontractors and workers. Subcontractors and their sub- ‘subcontractors will be responsible to submit all relevant safety training records of their staff and workers to HSO for record purposes. Filing, maintenance and discard of safety training records will be in accordance with the established quality records management for the Contract. 5.3 General Safety Requirement 1 Purpose To ensure all personnel know their obligations and responsibilities towards ‘safety, security and environmental control and fully comply with the rules and regulations governing the same. The safety rules and regulations state in unambiguous terms the “Dos” and “Don'ts” pertaining to the enforcement of safety, security and environmental control rules and regulations in order to achieve full compliance therewith, 5.3.2 General Safety Rules Itis the responsibility of all site staff, subcontractors and their sub-subcontractors, together with all workers, to strictly comply with the general safety rules to ensure safety while working at site. Revision: 3 Om Site Safety Management Plan Date: 19 Sept. 2019 =HmZ Page 150131 5.3.3 Emergency and Rescue Procedure Shimizu Corporation will develop site emergency response and rescue procedures before any work commences at site but in any event no later than. three months after the date of the Commencement of Works. These procedures will provide clear instructions to be followed in the event of an emergency, naming of responsible personnel, notification and co-operation proposal with appropriate rescue services and other authorities who would be involved, method of evacuation and other necessary measures. The procedure will be coordinated with local emergency services as required. Emergency procedures, giving full instructions and telephone number(s), will be posted at appropriate locations on site in Tagalog, English and any additional language as appropriate. Emergency drills including outside rescue and emergency services will be executed within one month of commencing construction and at six-month intervals. 5.3.4 Pul Convenience and Safety All works will be conducted so obstructions to traffic are minimized. Provision will be made for the safety and convenience of the public and the protection of persons and property. Shimizu Corporation will provide, erect and maintain barriers, barricades, lights, signals, signs and other control devices in accordance with the applicable iaws. Structural steel beams and other major structural components or heavy and potentially hazardous components will not be lifted and/or placed over roadways, bicycle paths or walkways that are open to the public. 5.3.5 Personal Protection Equipment Shimizu Corporation will ensure that all persons working at or visiting the site at least wear an approved type of safety helmet and certified appropriate footwear. Shimizu Corporation will assess all work areas and specify the appropriate personnel protection equipment (PPE) according to the applicable laws, standards and codes of practice. Shimizu Corporation will ensure that subcontractor's workers of any tier, including labour only, are issued with the specified PPE. All PPE will be properly maintained and replaced before the period of permitted use expires and shall be tested and approved by the DOLE based on its standards and or other means of verification. (New RA-11058-DOLE-DO-198 IRR sec. 8 paragraph 2) a.06/Dec/2018. Shimizu Corporation will ensure that all persons at the site use the appropriate PPE at all times. Training in its use will be provided where necessary. Failure to use such equipment will be considered as conduct prejudicial to safety and will be dealt with accordingly. The use of safely helmets at the site will be mandatory @ Revision: 3 Site Safety Management Plan Date: 19 Sept. 2018 Page 16 of 31 except for designated areas such as offices and canteens. Equipment selection and wearing requirements are determined from this assessment: (a) Safety glasses, goggles, or face shields will be worn in those areas where there is a reasonable probability of injury to the eye from flying particles, molten metal, chemicals/acids/caustics, or light radiation, or other eye hazards. (b) Head protection (hard hats) will be worn for protection from falling objects or work near energized electrical contact. (c) Foot protection will be worn where there is danger to the foot from falling/rolling objects, objects piercing the sole or electrical hazards. (4) Hand protection is required when hands are exposed to severe cuts/abrasions, chemical/thermal burns, or chemical absorption (e) Appropriate gloves, aprons, goggles, and boots will be used when necessary for protection against acids, other chemicals and other hazardous conditions which could injure employees. (f) Respiratory equipment in many cases is needed for protection against toxic and hazardous fumes/ dusts. (g) Supervisors must verify which equipment meets the need for breathing safety. 5.3.6 Control of Substances Hazardous to Health Shimizu Corporation will assess all materials for their occupational health and environmental capability. As far as possible or practicable, a less hazardous product will be used to replace any material that is toxic, explosive, carcinogenic, flammable or which may otherwise create a hazard. Where this cannot be done, Shimizu Corporation will conduct risk assessment and if appropriate, produce a method statement specifying the safe method of use or application which will be sent to the Engineer in advance for their statement of no objection. The Shimizu Corporation will maintain a material safety data sheet for all such material in the auditable health and safety records. 5.3.7 Alcohol and Non-Prescriptive Drugs Control will be established and maintained to prevent persons under the influence of alcohol and/or drugs entering and/or working at the site, No alcohol or non-prescription drugs will be allowed at the site. 5.3.8 First Aid All parts of the site where work is being carried out and the site offices will be provided with appropriately equipped first aid kits. First aid kits will be in sturdy weatherproof containers clearly identified in Tagalog, English and any additional Revision: 3 Om Site Safety Management Plan Date: 19 Sept. 2019, SHmZ Page 17 of 31 language as appropriate. Kits will be fixed in a visible and readily accessible location. All work sites will have readily accessible first aid facilities. These facilities will be able to provide immediate medical assistance for serious injuries and to deal with minor injuries. Shimizu Corporation will maintain an adequate number of personnel who are trained in basic first aid, 5.3.9 Fire Prevention ‘Shimizu Corporation will establish the following fire prevention strategy for all the work under its control: (a) All necessary and appropriate firefighting equipment and facilities will be provided to personnel, and they will be trained in firefighting. (b) Waring systems will be installed at all places where persons may be at risk from fire and all personnel will be given instruction in evacuation procedures, (c) All portable fire extinguishers will be visibly located, accessible and maintained in operating condition. Portable fire extinguishers will receive an annual service check and a monthly visual inspection. These will be documented on the tag on the extinguisher or other form. (d) All employees must know the location of firefighting equipment in the work area and have knowledge of its use and application. (e) Exits will be marked as such by a readily visible sign. Other doors likely to be mistaken for an exit will be marked as to their character or "Not an Exit" (f) Only approved safety containers shall be used for handling or storing flammable liquids in quantities greater than one gallon (4 liters). For one or less than one gallon (less than 4 liters), only the original container or a safety container shall be used. (g) When heat producing equipment is used, the work area must be kept clear of all fire hazards and all sources of potential fires will be eliminated. (h) Fire extinguishers will be available at all times when utlizing heat-producing ‘equipment or hot works doing spark potentials. (i) Smoke detectors shall be tested on a monthly basis on its functional condition. 5.3.10 Safe Operation of Plant and Equipment Alll plant and equipment will be safely operated by adhering to the following: (a) Operators for machinery - only qualified/trained/certified/licensed competent operators will be deployed for the operation of construction equipment and machineries to be used at site, (b) Lifting equipment: Revision: 3 om Site Safety Management Plan Date: 19 Sept. 2018 SHmz Page 18 of 31 (i) Only pulley blocks, chain blocks or sets of chain blocks, cranes, piling frames, material/passenger hoists, suspended scaffolds, chains, ropes, slings, rope sling, rings, hooks, shackles and swivels/eyebolts that possess a valid third party certificate of suitability issued by an approved | certified institution / personnel will be used at the site. (ii) All subcontractors will designate a site supervisor who will ensure any lifting operations are carried out safely. (iii) A competent / certified personnel will inspect all the cranes which are to be assembled at site for lifting activity. (c) Electrical equipment - all electrical equipment will be of the type approved by a qualified / electical personnel. (d) Portable electrical tools - all portable electrical tools to be used for construction purposes must be fully insulated / certified by a third party institution / personnel. (e) Machine/equipment guarding - safety flagmen will be provided to notify approach of all moving parts of machinery/equipment causing injury to any workers. (f) Gas oylinders (i) All gas cylinders will be secured in an upright position at all times to prevent them from falling over. (ii) Empty and filled gas cylinders that are not in use will be properly stored separately from the “live” cylinders. (iii) Provision of cylinder’s cap to empty or not in use gas cylinders. (iv) Suitable fire extinguishers will be provided for each set of oxy-acetylene or oxy-liquid propane gas (LPG) welding/cutting equipment. (g) Ladders: (i) Generally, ladders to be used at site are made of wooden materials of sufficient quality or metal for larger ladders. (ii) When using ladders, the top and bottom of the ladder will be properly secured to prevent slippage. 11 Communication and Signage All plant and equipment will be safely operated by adhering to the following procedures: Revision: 3 oO Site Safety Management Plan Date: 19 Sept. 2019 SHmZ Page 19 0f31 (a) General communication - all signs, labels, warnings, posters and safety information directed at people working on the site will be in Tagalog, English and any additional language as appropriate. Communication regarding any substandard safety items may be given verbally in the first instance but then be followed by written communication as deemed necessary and/or appropriate. (b) Electronic communication - Shimizu Corporation will ensure that radio and telephone communication is available at all times between all parts at the site and the site offices to ensure correct information. Such communication systems will provide a dedicated channel for use in case of emergencies involving accident, injuries, fatalities, serious damage or dangerous occurrence(s). 5.3.12 Temporary Access and Temporary Works ‘Shimizu Corporation will provide all necessary temporary access facilities, which will be constructed, installed and maintained in accordance with the applicable laws and codes of practice. In particular, ladders and any supporting structures will be regularly inspected to ensure that itis fit for purpose and free from defects. Any temporary access will be secured against movement, provide a good walking surface to prevent tripping hazards and incorporate fall protection measures. All temporary works will be checked in accordance with the applicable laws. and code of practice and the construction specification before use. 5.3.13 Safe Handling of the Materials Industrial hand gloves must be worn when handling materials where in contact with sharp edges, abrasive surfaces, slivers, chemicals etc. All circular objects must be suitably choked with chokers. All workers who are required to perform manual lifting tasks will be reminded on: (a) Back care and correct lifting method when doing manual handling. (b) Hands and feet to be keep clear of pinch points. (c) The position of operators when using pinch bars (so that tools don't slip or falll to cause injury). 5.3.14 Work at Height All work at height in excess of 2m will be executed within a properly secured and otherwise protected area. If such protection is impracticable, work will only be permitted with the use of well secured safety harnesses and life lines. All personnel in such locations must wear approved and tested safety harnesses at all times secured to Ife lines. Site Safety Management Plan Date: 19 Sent 2019 Page 200131 5.3.15 Excavation Where any construction worker engaged in excavation works is exposed to a hazard of falling or sliding material or article from any bank or side of such excavation which is more than 1.5m above his footing the worker is to be protected by adequate bracing against such a bank or side. Where banks of an excavation are undercut, adequate shoring is to be provided to support the material or article overhanging such a bank. Excavated material will be stored at least 1.0 m. from the edge of an open excavation or trench and the banks of such excavation or trench will be stripped of loose rocks and other materials which may slide or roll or fall upon any worker working below the bank. Metal ladders and staircases or ramps, as the case may be will be provided for safe access to and egress from excavations where the depth of such excavation exceeds 1.5m. Such ladder(s), staircase(s) or ramp(s) will comply with DOLE / Bureau of Working Condition / Standards as appropriate. If the depth of a trench and/or other excavation exceeds 1.5m, suitable and appropriate protection measures will be implemented to prevent persons falling thereinto. Where the depth of a trench and/or excavation exceeds 4m, such protection measures will be improved and be in accordance with the design and drawing of a professional engineer. Shimizu Corporation will ensure that suitable warning sign or notices, required for the safety of workers carrying out excavation works, will be displayed at visible places in, English and any additional language as appropriate. HSO to ensure that a gas test will be conducted at the deep excavation before any entry of workers. An excavation permit shall also be prepared and approved before start of any work excavation, 1.0 m deep. 5.3.16 Health and Safety Publ y Shimizu Corporation will ensure that health and safety matters are given a high level of publicity at the site. Safety Posters and Safety Signs written in Tagalog, English and any additional language as appropriate which draws attention to site safety, rescue and occupational health will be made or obtained from appropriate sources and will be displayed prominently in relevant areas of the site. In addition, a safely campaign will also be facilitated at work site to enhance safety awareness to all workers. 5.3.17 Cleanliness of the Workplace Measures will be taken to ensure all building materials are stored or stacked in a safe and orderly manner without obstructing passageways or place of work and will not be placed near to edges of structures or working platforms. Any debris generated which is highly flammable/combustible in nature will be removed immediately from the site. Site Safety Management Plan Date: 10 Sept 2019 Pope 2131 Walkways, staircases, scaffolds, roof tops and working platforms will be cleared of construction debris so as not to cause obstruction and hazard to workers. Containers will be provided at designated locations for the collection/disposal of debris and rubbish. HSO to ensure that housekeeping will be absolutely implemented at the work site. 5.3.18 Safety Rules and Regulations for all Workers "Safety Rules and Regulations for All Workers" will be posted on all site safety notice boards. An example of the safety rules is incorporated as Appendix 4. 5.3.19 Noise All necessary procedures will be established and maintained by Shimizu Corporation to prevent people working at the site or members of the public in the area being exposed to unreasonable levels of noise. Details of this procedure are explained in the separate Outline Environmental Plan. Where necessary provision of suitable ear protection is required / provided. 20 Shimizu Corporat n Plant, Equipment and Vehicles Alll construction plant, equipment and vehicles used by Shimizu Corporation will be inspected and tested as appropriate. Each vehicle, item of plant and equipment will be given a unique identity number that will be clearly displayed in the machine at all times. Such plant, equipment and vehicles will fully comply with the applicable laws and be properly registered and certified and will be maintained in line with the manufacturer's specifications. Copies of all such registrations, certificates and maintenance records, etc, will be kept in the safety-related documentation files. Shimizu Corporation will also establish a system to control the delivery of short term hired vehicles and plant. 5.3.21 Shimizu Corporation Employees All employees of Shimizu Corporation and all subcontractors of every tier, including labor, only employees and all employees of other contractors who are working within the site will at all times wear suitable identification and display an entry / visitors permit. The said permit will be in Tagalog and English, dated and clearly show the person’s name and his employer. Shimizu Corporation will establish procedures to control the entry of visitors into the site. Visitors include all those who are not regularly on site and who have not undergone an initial safety briefing. All visitors will be issued with appropriate permits for entry. 5.3.22 Traffic Management This section shall be described in detail and will be submitted separately. Revision: 3 om Site Safety Management Plan Date: 19Sept_ 2019 sHmz Page 22 of 31 5.4 Particular Safety Requirements Shimizu Corporation will comply with the particular requirements for work on or adjacent to the LRT Line in reference to Section VI-2 Technical Requirements, Sub- Clause 4.6 and Sub-Clause 4.7 General Specifications of the Bidding Documents. 6.0 GROUP MEETINGS 6.1 Purpose The purpose of group meetings is to assemble all personnel who are responsible for safety so that they may formally address the safety and occupational health issues and take appropriate actions to achieve work site safety management objectives. 6.2 Construction Progress Coordination Meetings This meeting will be chaired by the PM or his appointed deputy and will be held every week, or as required. Site engineers, supervisors, safety personnel, subcontractors and other personnel as determined by the PM will attend the meeting Besides discussing any problems detected in the engineering and construction interface, the meeting will take the opportunity to discuss and co-ordinate safety matters in order to: (a) Obtain safe working conditions. (b) Eliminate congested work conditions among subcontractors. (0) Use the work areas safely and effectively. (d) Demonstrate Leadership and Commitment. The PM will take the lead in addressing these issues and any follow up matters. Shimizu Corporation's site safety personnel will attend the meeting in the capacity of advising the members on the proper corrective measures. The agenda and list of all attendees will be kept in the site office for record purposes. 6.3 Site Safety Committee Meetings It is the responsibility of the PM to establish a monthly site safety committee meeting schedule and ensure that the committee meets at least once a month to discuss safety issues related to the site. The PM, as the chairman of the committee, will preside on all meetings with the HSO as the secretary of the committee The HSO will ensure that all safely representatives appointed by the subcontractors are official and have authority to make decisions and are knowledgeable in carrying out their safety duties. The Engineer's staff will be invited to sit in on the committee on an ex-officio basis. Itwill be the HSO responsibility to ensure that the minutes of the meeting are kept at the site office and copies distributed to all the committee members. Matters arising, Chairman's review of SSMP performance/condition Safety report reviews. Site safety inspection report. Accidents and incidents. Report from the Engineer. Any other business. Femeaogp 6.4 Daily Toolbox Meeting The purpose of these meetings is to brief the subcontractors with the method statement, work procedures and risk assessment for the respective tasks they will be undertaking on a daily basis and also to inform them about the relevant safety rules and regulations. The subcontractor will have all work groups conduct the tool box meeting daily. Each respective work group supervisor(s) will conduct the tool box meeting with the participation of respective Shimizu Corporation's site supervisor & safety personnel to observe and ensure that toolbox meeting is conducted proper! The HSO will monitor from time to time any particular Toolbox Meeting's Topics that may include matters raised by Shimizu Corporation The meeting record form will be used to record the meeting and will be submitted to the HSO who will maintain the records. The Toolbox Meeting can include the following but not limited to: (a) A short prayer and a minute exercise. (b) Tasks for the day, their existing and potential hazards, and the necessary controls, (c) PPE inspection (appropriate to the tasks) and reminders on care and maintenance (d) Highlights of incidents (near miss, accident, and/or potential incidents) that occurred the previous day. Take the lessons learned and discuss. (e) Safety provisions check: Fire extinguisher, barricades, warning signs, first aid supplies / equipment, ete. (f) Reminders on Fire Protection and Emergency Response Plan Revision: 3 Om Site Safety Management Plan Date: 19 Sept. 2019 samz Page 24 of31 7.0 SAFETY INSPECTION 7.4 Purpose To verify that safety provisions and work practices conform with the site safety rules and regulations and relevant statutory requirements. To assess hazardous acts and conditions associated with the work implementations at workplace so to ensure and undertake appropriate controls and measures 7.2 Inspection of Electrical Equipment, Vehicles and Other Equipment Itis the responsibility of the HSO to ensure that the following statutory inspections are carried out: (a) Electrical equipment, heavy equipment, vehicles and etc. (b) Scaffold inspection. (c) Formwork inspection before the placing of concrete, The HSO is to ensure that records of all the above inspections are properly documented and kept in the site office for reference. Shimizu Corporation will submit a written report to the ER after each regular inspection. 7.3 In - House Safety Inspection Programme It is the responsibility of the HSO and his safety supervisors, including all site Supervisors, to conduct regular safety, health and environment inspections to ensure that all works are carried out in a safe manner and that all unsafe conditions and acts are discovered and rectified accordingly. An effective in-house safety, health and environmental inspection programme will be implemented at site to be conducted by the relevant competent safety inspectors. Check lists will be formulated to ensure that the inspections are comprehensive and thorough The safety, health and environmental inspections will be carried out at such frequencies as to suit the dynamic nature of the construction activities and to ensure that all work activities are effectively covered. 7.4 Workplace Safety and Health Audit In compliance with workplace safety and health regulations, Shimizu Corporation will arrange for regular safety and health audits at least once every three months. The audit will be conducted by an Audit Team formed by Shimizu Corporation’s Main Office. The Audit Team formed by Shimizu Corporation will be approved by the Engineer. The audit team will be required to develop audit methodology including the detailed resumes of their audit team and audit check lists subject to the Engineer approval before commencement of any audits. Revision: 3 (2) Site Safety Management Plan Date: 19 Sept. 2019 sHmz Page 25 of 31 The scope of the safety audits will cover all or any of the following: (a) Safety and health management system of the workplace. (b) Any risk assessment relating to the workplace or the work carried out. (c) Any work process at the workplace. The documentation generated by the audit process, including score sheets, will be made available to the Engineer. 7.5 Daily Safety Patrol Site Engineers/Supervisors in-charge of particular work will conduct daily safety patrols to check any unsafe acts/conditions and, if observed, to order the relevant ‘subcontractor/worker to carry out immediate corrective actions. The HSO will monitor the Daily Safety Patrols and ensure that the personnel involved carry out their tasks responsibly and effectively. Supervisors/foremen are responsible for preparing and submitting observation and daily safety reports to the HSO for his review. The latter will comment and make recommendations to correct any reported unsafe acts/conditions. The SO will summarize all the site supervisors’ daily safety reports into an overall safety report and submi to the PM and to the construction manager who will ensure that corrective actions are carried out where necessary and/or deemed appropriate. 8.0 SITE INVESTIGATION AND ANALYSIS 8.1 Purpose The purpose of incident investigation and analysis is to identify the causes of accidents, incidents, dangerous occurrences and near misses to prevent recurrence. The following definitions are used during cident investigation and analysis: Incident : Any undesired condition that results or could have resulted in harm to people, damage to property, the environment or loss of properties. This includes “near accidents” and “dangerous occurrences’. Accident ‘An incident involving actual loss through injury, damage to assets or environmental threat/harm Near Miss : An incident which in other circumstances, could have resulted in loss through injury, damage to assets or environmental harm. Dangerous : An occurrence that may endanger persons, e.g. toppling Occurrence of crane, excavation collapse, failure of hoist, fire, explosion etc. @ Revision: 3 Site Safety Management Plan Date: 19 Sept. 2019, Page 26 of 31 Major Incident Serious Injury Minor Injury Lost Time Injury Accident Frequency Rate Incident Severity Rate The criteria in classifying major incidents are: (i) All fatalities involving employees, contractors or third parties. (iv) Suspension of work for more than 24 hours, Any injury resulting in: ) Fracture of any bone. (i) Loss of sight or an eye (permanent disability). (ii) Any other injury (e.g. chemical burns, inhalation of toxic vapours), excluding occupational disease, which results in the person being hospitalized for more than 24 hours, unless detained solely for medical observation. Any injury that does not cause the injured person to lose normal job time beyond the day or shift upon which the injury occurs. Any injury that causes the injured person to lose normal job time beyond the day or shift upon which the injury occurs (eg, hospitalized for more than 24 hours or medically unfit for at least three working days). The ratio of the number of incidents per million manhours worked. The ratio of the number of man-days lost due to incidents per million man-hours worked. 8.3 Accident Reporting, Investigation and Analysis Process If any accident occurs, it must be reported to the safety team and the PM. The safety team will apply the initial first aid and the injured person referred to the nearest hospital as soon as possible. The relevant site supervisor with the safety team will investigate the cause of accident and implement corrective and prevention action. The SO and PM will review the corrective and preventive action. 8.4 Accident Reporting Procedure Shimizu Corporation's and the subcontractor’s employees will report immediately any incident that they are involved in or witnessed to their immediate supervisor. The supervisor in-charge, on receiving an accident report, will inform his immediate superior and the safety officer. The Engineer will be notified of any accident, incident, dangerous occurrence or near miss associated with the contract. For major accidents, the HSO will submit an incident report to the relevant authorities. 8.5 Accident Investigation Procedure The site supervisor will ensure that no equipment, material or other evidence of the incident is removed from the scene unless it is essential to do so in order to prevent further injury or damage, and carry out an initial investigation, The site supervisor will write an accident report on the accident occurrence and submit it to the safety officer and HSO for review. The HSO will evaluate the supervisor's investigation report and ensure that it adequately addresses: (a) Facts and circumstances of the incident. (b) Causes of the incident. (c) Recommended corrective actions to prevent recurrence 8.6 In Procedure lent Analy: Incident analysis will involve a systematic process of identifying the direct cause, root cause and weakness in the safety management system, to prevent similar types of incidents from recurring. The direct causes are defined to include unsafe conditions and unsafe practices and include: (a) Unsafe conditions: (@ Inadequate or missing guards. Gil) Defective tools, equipment or materials and poor housekeeping. (ii) Unsafe construction, inadequate illumination or improper/inadequate ventilation. (b) Unsafe practices: (®) Failure to use PPE/improper use of PPE, (ii) Using defective equipment, improper loading or carelessness. (iii) Horseplay. (iv) Operating without authority/training/supervision. Root causes are defined to include work and human factors. The root causes are then analyzed to identify the weaknesses in the safety management system. Itis the responsibility of the HSO to conduct an accident/incident trend analysis to monitor the trend of incident occurrence. This will assist his team in implementing strategies to prevent recurrence of similar incidents. Om Site Safety Management Plan Date: 19 Ser. 2019 sHmz Poge 28 of 31 8.7 Corrective Actions For each identified SSMP system failure, a corrective action will be implemented. For each identified direct cause (unsafe act or unsafe condition) corrective action(s) will be implemented. Whenever practical, all accident investigations reports will be reviewed by the SO. He will re-examine the evidence supplied and determine whether all likely causes have been identified. The HSO will carefully consider whether the management system failures identified are sufficiently complete. The safety committee will also monitor and ensure that the recommended solutions are fully implemented during a later audit of corrective actions. It is the responsibility of the PM to assign staff to implement the recommendations within a stipulated deadline. These recommendations are intended to prevent recurrence of incidents. All supervisors will inform all workers of these measures and ensure that they are implemented, The status of implementation of the recommendations will be regularly reviewed during the safety meetings until appropriate closure of all corrective actions. 9.0 SAFETY RELATED DOCUMENTATION All safety related documentation including permits, certificates, reports and records will be kept on site in a readily accessible location using an auditable indexed system. 9.1 Site Safety ry The safety supervisor will maintain a daily site safety diary. It will comprehensively record all matters pertaining to safety including, but not limited to, record of inspections, audits, injuries, dangerous occurrences and safety violations. This will include, but is not limited to, the overturning, collapse or failure of any lifting equipment or structure. Initial notification may be verbal but will, in every event, be followed by a detailed resolution report. Riggers carried-out and shall enter the results of such tests in a logbook or log- sheet; Shall report any failure or malfunction of the crane to the site supervisor and shall make an appropriate entry in the logbook or log-sheet of the crane condition; Shall not hoist any object if in doubt or ascertain the weight of the object; Shall not hoist any load if there is any obstruction in the path of maneuver (if any such obstruction exists, shall report this immediately to the site supervisor); Shall not hoist any load unless received a clear signal from a signalman; Shall not operate a mobile crane or a tower crane within 3 meters of any live overhead power line. Their responsibilities and duties include the following: Check the slings to be used for slinging the loads are of good condition, sound and suitable material with adequate strength and free from patent defect; Ensure that an adequate number of legs of the sling are used and that the slinging angle is correct to prevent the sling from overloading during the hoisting operation; Ascertain the weight of the load to be lifted and inform the crane operator of the weight of the load; Ensure that only proper lifting gears are used in conjunction with the sling; Ensure that tag lines are available to prevent swinging movement of the load to be lifted; Ensure that any loose load including stones, bricks or tiles are placed in a receptacle to prevent the load or part thereof from falling during the lifting operation; Place adequate padding at the edges of the load which are in contact with the sling to prevent damage on the sling; Report any defects of the lifting gears to the site supervisor. 9. Signalmen Their responsibilities and duties include the following: Ensure or verify with the rigger that the load is properly rigged-up before giving a clear signal to the crane operator to lift the load; Give correct and clear signals to guide the crane operator in the maneuver of the load safely to its destination.
