If you answered “yes” to all of the questions, please follow the five easy steps below:
Complete the Nonimmigrant Visa Electronic Application (DS-160) form. After completing the form, print and save
your confirmation page.
Please Note: Using the Interview Waiver service does not guarantee visa issuance. In some cases, you may
be required to appear for a visa interview at the U.S. Embassy. If you submit an incomplete and/or
inaccurate application, or if you do not meet the criteria listed above, we will return your application via the
courier, and you will be asked to schedule an interview. You should submit your application well in advance
of your planned travel, to allow for the possibility that a visa interview will be necessary.

21. Have you ever been refused for a U.S. Visa application, or been refused admission to the
United States, or withdrawn your application for admission at the port of entry? Explain.
22. Has anyone ever filed an immigrant petition on your behalf with the United States
Citizenship and Immigration Services? Explain.
23. Complete Home Address (house number, street, unit, province, zip code)
