NCM101 Health Assessment


SKILLS LAB
Level Outcome:
At the end of this course, the First year NCM 101 nursing skills students will be able to
perform safe skills that deals with the priciples and technique, given simulated situations in
selected settings, rendering safe and appropriate care utilizing the nursing process.
Learning Outcome:
At the end of this course, the third year NCM 101 nursing skills students will be able to:
1. Systematically and proficiently perform a physical examination of the Neck on adults.
2. Correlate physical examination findings with anatomic and physiologic characteristics of
the Neck
Objectives:
During the performance of the activity in Assessing the Neck, the nursing students will be able
to:
1. Describe the common methods of physical examination of the Neck.
2. Gather essential data from examination of the Neck
3. Perform the procedure with confidence.

General Instructions:
1. Attend the scheduled skills laboratory class wearing a prescribed school uniform
2. Review the skill(s) to be practiced and demonstrated
3. Secure a copy of checklist provided for skills demonstration
4. Gather the equipment neede for skill performance
5. Students will follow safety measures at all times
6. No food or drink is allowed during the online clinical skills class
7. Secure a convenient place where you can perform the procedure to avoid distractions

Return demonstration in assessing the Neck will on Wednesday 7-11am, 3-5pm during our
online/F2Fclass. Each student will be given a 2 to 5 minutes to perform the procedure. Video
recording during the return demonstration will be automatically saved to google drive as basis of
grading.

Assessing the Neck

1 Introduce yourself, and verify the client’s identity.
Explain to the client what you are going to do, why it is necessary,
and how she can cooperate
2 Perform hand hygiene, and observe other appropriate infection
control procedures
3 Identify the patient
Provide for client privacy
4 Inquire if the client has any history of the following:
 Any problems with neck lumps
 Neck pain or stiffness
 When and how any lumps occured
 Any diagnoses of thyroid problems
 Any treatments such as surgery or radiations
5 Position the client comfortably- seated, if possible.
ASSESSMENT
Neck Muscles
6 Inspect the neck muscles (sternocleidomastoid and trapezius0 for abnormal
swellings or masses.
7 Ask the client to hold head erect
8 Observe head movement. Ask the client to:
9 Move chin to the chest ( determines function of the
sternocleidomastoid muscle)
1 Move headback so that the chin points upward ( determines function
0 of the trapezius muscle).
1 Move head so that the ear is moved toward the shoulder on each
1 side ( detremines function of the sternocleidomastoid muscle)
1 Turn head to the right and to the left ( determines function of the
2 sternocleidomastoid muscle)
1 Assess muscle strenght. Ask the client to:

1 Turn head to one side against the resistance of your hand. Repeat
4 with other side.
1 Shrug shoulders against the resistance of your hands.

Lymph Nodes
1 Palpate the entire neck for enlarged lymp nodes.

Trachea

Salivary Glands
1 Place your fingertip or thumb on the trachea in the suprasternal
7 notch, then move your finger laterally to the left and the right in
spaces bordered by the clavicle, the anterior aspect of the
sternocleidomastoid muscle, and the trachea.
Thyroid Gland

1 Inspect the thyroid gland

1 Stand in front of the client

2 Observe the lower half of the neck overlying the thyroid gland for
0 symetry and visible masses.
2 Ask the client to hyperextend head and swallow. If necessary, offer a
1 glass of water to make it easier for the client to swallow.
2 Palpate the thyroid gland for smothness. Note any areas of enlargement, masses, or
2 nodules,
2 If enlargement of the gland is suspected.

2 Auscultate over the thyroid area for a bruit

2 Use the bell-shaped diaphragm of the stethoscope

2 Documment finding in the client record
