With reference to the above subject,please note:

In this regard please note that :

We are in receipt of your letter alongwith below mentioned

documents sent by you for issue of duplicate share certificates
and change of your name.

- An Indemnity bond on stamp duty of Rs.500/- duly notarised.

- An Affidavit on stamp duty of Rs.100/- duly notarised.
- Copy of police complaint for loss of certificate.
- Your signature attested by bankers.
- An Affidavit on stamp duty of Rs.100/- duly notarised for
change of signature.
- Notarised copy of your Aadhaar card.
- An Affidavit on stamp duty of Rs.50/- duly notarised for loss
of certificate.

In this connection, please note that the documents sent by you is

not in order hence the following documents/corrections are
required to be made:

1. Application form not enclosed by you. Please fill the

Application of issue of duplicate share certificate (draft
enclosed).

2. Indemnity Bond :
- Certificate no. of bonus share is mentioned as 930307 insead of
939307. Please correct the same.

- Indemnity bond is not signed by you on all the pages. Please

put your signature on all the pages.

- Date and place are not mentioned in indemnity bond. Please

mention the same.

- Name, address and signature of both the witnesses are missing.

- Name and signature of surety is missing. ** Indemnity is not

as per our prescribed format. Please correct the same as per the
enclosed format of Indemnity Bond and send the same duly signed
and notarised. Please note that execution date of an Affidavit
should be on or after the date of execution of an Indemnity Bond.

- Pin code no. mentioned on the Indemnity bond, Affidavit

i.e.(370205), and Aadhaar card, police complaint is different
i.e. (370110). You are therefore requested to mentioned the
correct address on all the documents with the revelant address
proof.

3. Affidavit :

- Affidavit is not signed by you on first page. Please put your

signature on all the pages.

4. Copy of police complaint / FIR :

- You have mentioned the old certificate details in copy of

police complaint for loss of certificate. Please lodge the fresh
FIR with all the following details mentioned in it. Please
note, certified true copy of FIR or acknowledgement of police
complaint is required.

equal to market value that of shares as on date of execution

of surety duly attested by the bank manager Or Notary Public

surety for spouse/family member/shareholder.

- Please submitt the self attested copy of proof of

income/bussiness/property of surety.

6. Affidavit for change of signature :

- Distinctive no. of subdivided share is mentioned as 17748376

insead of 17748386. Please correct the same.

- Affidavit is not signed by you on all the pages. Please put

your signature on all the pages.

7. Please submitt the self attested copy of your PAN card and
original cancelled cheque bearing your name printed on it.

8. Please submitt the self attested copies of PAN card and

address proof of both witnesses.

Hence, we are returning the aforesaid documents without taking

any action. Please send the above mentioned required documents
duly rectified.

** We have marked with pencil wherever correction has to be made

for your ready reference. Please put your signature wherever the
corrections have been made.

On receipt of the above documents after clearing the objections,

we shall proceed further in the matter.
