Dear Ms. Anderson

I am writing regarding Ms. Ryan, who had abdominal exploration surgery today.
She needs to be transferred to your unit, as your further monitoring and care
would be appreciated.

During the operation, Ms. Ryan had uncontrolled bleeding, resulting in a large
amount of blood being lost. For which she received 2 units of PRBCs and 1 unit of
plasma.

Post-operatively, her vital signs were stable, but she woke up reporting some
pain, for which she was given paracetamol and ketorolac. She is conscious at the
moment and will be transferred with a central line, a urinary catheter, and two
abdominal drains attached to a collecting bag.

Kindly monitor Ms. Ryan’s vital signs, urine output, as it must be calculated every
12 hours, and her drains regularly. And please inform your resident doctor if you
notice any abnormal findings, such as abnormal vital signs, an unusual amount of
blood in drains, or low urine output. Please note that she has a midline incision
that was closed with staples, so keep it sterile and dry. Her CVC must be kept
sterilized, and please change the dressing daily. Please note that she keeps on
wearing her compression elastic stocking.
