B. Formulate two (2) Nutrition Diagnosis following the PES format and provide a
scientific explanation for each (12 points). May provide references for the scientific
explanation.

● Nutrition Diagnosis #1: Excessive intake of protein related to multiple


allergies or malabsorption syndrome from a unknown protein source and
restricted foods that contains milk products as manifested by BMI of 18.4,
tiredness, exhibit GI distress, and mild diarrhea after meals

o Scientific explanation (how is the nutrition diagnosis related to the case of


the client)

Allergies can also manifest as uncomfortable gastrointestinal symptoms.


The same signs and symptoms of a food allergy, such as nausea,
vomiting, cramps, and diarrhea, might also be brought on by a food
intolerance or an allergic reaction to anything else you ate. It was
mentioned in relation to the case of the 50-year-old patient she was at risk
of developing a milk allergy. Having said that, a milk allergy is a sort of
food allergy that develops when your immune system unintentionally
develops a defensive reaction to milk proteins. This is due to an allergic
reaction. The causative was she went to a restaurant but was unaware
that her food had hidden milk production on it which resulted in mild
diarrhea and other symptoms indicated. Her weight, which is 114 for a 5'6"
height, is also provided. It's regarded as being underweight. 'Failure to
Thrive' (FTT), one of the symptoms of a cow milk allergy, may be
diagnosed as a result of slow weight increase. It might be challenging for a
patient who is diagnosed with milk allergy to receive the proper amount of
essential vitamins and minerals and this could result in unhealthy weight
loss. And lastly, tiredness or fatigue experienced by the patient. Along with
immunoglobulin E, histamine is also released by the immune system. Both
of these substances can cause an inflammatory response in the body,
which can lead to tiredness in addition to other, more evident allergic
symptoms.

● Nutrition Diagnosis #2: Food, Nutrition, and Nutrition-related Knowledge

understanding of the requirement for food and nutrition recommendations.

Given that she mentioned having a variety diet plans, I believe she still
lacked sufficient knowledge of the foods she ought to avoid, was on the
wrong diet, lacked the skills necessary to read food labels, as she
mentioned, and neglected to ask about the ingredients which renders her
copies ineffective and causes her allergy to trigger. No matter how tiny the
amount, the key foods or substances are, this may cause significant
adverse reactions. Aside from that, inadvertent ingestion might cause an
allergic reaction, which in certain cases can be extremely severe and even
dangerous.

C. Nutrition problems are identified using assessment evidence, and the nutrition
diagnosis is the link between assessment and intervention. Work on these
premises and with consideration of the assessment and diagnosis you have formulated,
you are to identify (1) goal appropriate to resolve or improve the nutrition
diagnosis/problem by provision of nutrition education and identify three (3) nutrition
interventions under the set goal and provide the means of monitoring and evaluation
for each of the identified interventions (18 points).

● For Nutrition Diagnosis #1

o GOAL: The patient will be more careful and accurately demonstrate how
to recognize and distinguish allergens in food

o Nutrition Interventions:

1. Provide health teaching to the patient in layman’s terms on the


effect of milk/protein and its effect on the client’s health and
well-being.

2. Educate the client on what food/groups are high or may have


milk/protien and recommend inexpensive safer/vegan
alternatives.

3. Educate the client on how to properly identify and read food


labels correctly.

o Nutrition Monitoring/Evaluation:

1. The patient will understand and verbalize the effect of


milk/protien on their health and well-being.

2. The patient will correctly identify food/groups that has
milk/protien and on food labels (such as lactose monohydrate,
milk sugar, milk ingredients, milk protein, whey, and more)

3. The patient will avoid milk/protien rich food (such as


lactose-containing food/groups including, milk, cheese, yogurt,
and more)

● For Nutrition Diagnosis #2

o GOAL: The patient will have efficient knowledge of the food she
consumes and of the possible consequences.

o Nutrition Interventions:

1. Educate the patient on how it is important to ask about the food


ingredients when eating at restaurants to avoid consuming hidden
milk products.

2. Provide patient education on how to read food labels and proper


diet plans.

3. Educate the patient regarding the possible consequences of


consuming milk products despite being allergic.

o Nutrition Monitoring/Evaluation:

1. The patient will be able to ask about the ingredients to avoid foods
she is allergic to. (Specifically, milk products)

2. The patient will be able to read food labels and understand correct
diet plans (Identify the serving information, total calories per serving
and container, daily value, and check if it contains milk).

3. The patient will be aware of the possible consequences when milk


products are consumed. (Allergic reaction includes swelling or
airways, impairing the ability to breath, and a sudden drop in blood
pressure causing dizziness and fainting).
