request or the termination of my Autosweep RFID Account.

 I have read and clearly understood the Autosweep RFID Subscription Agreement and I agree and consent to be bound by the terms and
conditions of the Autosweep RFID Subscription Agreement, including any amendments thereto, for all vehicles enrolled under my RFID
Account, including the above-listed vehicles.
