order to complete this survey for our research project, we need your assistance. This study’s goal
is to investigate how students’ athletic participation affects their academic performance at the
Asian Institute of Technology Digos Inc.

This data will only be utilized for the research paper we are writing and for this study. Your
replies are completely secret, and neither your coach nor the athletic department will receive a
copy. We appreciate you taking a few moments out of your busy day to assist us with our
