1126 The Transverse Plane Deformity of Structural Scoliosis R. M. SMITH, FRCS," R. D. POOL, FRCS,+ W. P. BUTT, FRCR, FRCP(C), FACR(Hon),¢ and R. A. DICKSON, MA, ChM, FRCSt ‘An examination of the transverse plane aspect of de- formed vertebrae from specimens of both human and animal scoliosis identified a consistent pattern of int Vertebral deformity. In the animal model, dynamic bor growth studies illustrated bone drift in the opposit tion to the rotation of the scoliosis, suggesting that the bone growth in the transverse plane was attempting to ‘correct rather than produce the deformity. The observed pattern of vertebral deformity can be explained in simple terms because it obeys the standard laws of bone growth and remodeling. This observed growth patter is consis- tent only with the production of idiopathic scoliosis by the rotation of a primary lordosis. [Key words: scoliosis, transverse plane deformity, vertebral growth, bone drift, primary lordosis] DIOPATHIC STRUCTURAL SCOLIOSIS isa complex three-dimensional Je associated with changes inthe shape of the spine in the sagittal, coronal, and transverse planes. Although the deformity of structural scoliosis with respect 10 the whole spine was accurately described over 120 years ago," its true nature has only been appreciated sporadically since then.*'©"79 The overall deformity is one of Jordosis, lateral curvature, and rotation In idiopathic structural seotio- sis, the direction of the rotation is always with the apical verter bodies toward the convex side of the deformity. This rotation is the essential feature that distinguishes. structural from nonstructural scoliosis.” ‘The deformity at the vertebral level is less obvious. Others* estb- lished thatthe sagital plane deformity of lordoss is atthe bony level, and vertebral wedging inthe coronal plane is widely accepted." The asymmetry of scoliotic vertebrae in the transverse plane was frst described by Dods* and Bampfielé? in 1824. More recent accounts, "2122 however, have added little new information and hhave not produced a consistent description of this element of the
