3.O SCOPE OF WORK

GCS Cont. Co. Ltd. will construct the temporary facilities for TECHNICAS REUNICADAS.
As required for the execution of the Temporary facilities (T-2020 JUBAIL EXPORT
REFINERY –TEMPORARY). The main elements of the work are the following:

The scope of works is to construct camp, consist for four areas, for which
includes, Satellite field offices, Main site office, and Lay down area.

¾ Satellite Field office scope of works which consist of Utilities such as,
electrical system, communication system, firewater lines system and
sewer system .Site works includes, Construction of boundary fence and
Gates, asphalting the roads and car parking including their
shelter.Building which consist of company Satellite office, Contractor
Satellite office TR1, Contractor Satellite Office TR2, First Aid clinic, Prayer
room, smoking shelter and Guard house.

¾ Main Site Office scope of works which consist of Site works ,Utilities
and Fabrication and Installation .Site works which includes Backfilling and
compaction of marl, construction of boundary fence and Gate, asphalting
works includes roads and parking area with own car shelter. Utilities are
composed of electrical works, communication system, water system and
fire network system. Building which consist of Company Main Office,
Contractor Main Office TR1 ,Contractor Main site office Tr2 ,smoking
shelter and Guardhouse ,scope of works shall includes fabrication and
installation including inside utilities of Portacabin and including Testing
and commissioning works.

¾ Lay down Area scope of works which consist of Utilities such as,
Electrical Distribution system, water system,Fire network system. Building
includes construction of Warehouse for contractor and their office
management which consist of each own utilities for warehouse and Office
management.

6.0 LOSS PREVENTION PROGRAM AND HAZARD IDENTIFICATION PLAN (HIP)

The contractor has identified the work hazards, their locations and their classifications as
per Saudi Aramco Construction Manual:-

6.1 CLASS ‘A’ HAZARD


The condition or circumstances likely to cause permanent disability, loss of life or limb
and/or extensive loss of property, equipment or materials to Saudi Aramco, to GCS
Contracting Co. Ltd or to the general public.

1. LOCATION

The area likely to be exposed to Class ‘A’ Hazards are as follows:-

a) Handling, transportation and installation or assembly of heavy items such as


structural steel, pre cast and etc. that requires the use of lifting equipments
such as crane, hoist, forklift, boom truck etc.
b) Testing and commissioning stage of electrical and HVAC systems.
c) Working in a vicinity of an overhead transmission lines or underground
cables.
d) Working in an enclosed area with limited access and ventilation.

2. PREVETIVE AND CORRECTIVE MEASURES

Prior to working in the above mentioned locations, the following precautions shall be
strictly followed to avoid unforeseen injuries to persons or damage to property and
equipment.

a). All driver and/or operators of any transportation or heavy equipments must
have in their possession at all times a valid Saudi Government driving license for
class and type of vehicle/equipment they are driving/operating. All equipment
operators must have Saudi Aramco Operating Certificate.

b). Testing and commissioning of all electrical equipment, transformers, HAVC,


etc will be done in a safe manner as well.

c). Safety site meeting will be conducted at least once a week or as the situation
calls for in order to explain to the workforce in the site the possible presence of
hazards or likely to be encountered during the progress of that particular work.

6.2 CLASS ‘B’ HAZARDS

A condition or practice likely to cause serious injury or illness (resulting in temporary


disability) or property that is disruptive, but less sever than Class ‘A’

1. LOCATION

a). Construction site


b). Workshops
c). Asphalting and paving operation for the construction areas and the roads.

2. PREVENTIVE AND CORRECTIVE MEASURES

a). Before commencing on any work activities on site, work permit shall be
obtained first from Saudi Aramco authorized representative.

b). All personnel working in the construction site must wear the required Personal
Protective Equipment (PPE) to void possible injuries.

c). Power supply connections for the electrically operated tools will be carried out
by the authorized electrician, which will be 110V then checked by Electrical
Engineer.

6.3 CLASS ‘C’ HAZARDS

A condition or practice likely to cause minor (non-disabling) injury or illness or non-


disruptive property damage.

1. LOCATION

These types of hazards are normally possible in the office areas and store areas of
the contractor as well as Saudi Aramco field offices and material storage yards.

2. CORRECTIVE AND SAFETY MESUSRES

a). Provide fire extinguishers and fire water buckets in a specific location easily
accessible in case of fire.

b). Store materials in an orderly manner and with sufficient ventilations.

c). Maintain proper and good house keeping.

d). Paint warning signs and directions around areas that will likely cause accidents.

7.0) ASSIGNMENT OF SAFETY RESPONSIBILITIES

7.1 INDIVIDUAL RESPONSIBILITES

1. To co-operate in implementing the requirements of all Loss Prevention Program,


its related codes of Practice and Safety Instructions. Their responsibilities in this
field are no less than for any other function.
2. To refrain from doing anything which constitutes a danger to themselves or others?
3. To immediately bring to the attention of their line management/supervision any
situations or practices that may lead to injuries or ill-health.

7.2 COMPANY SAFETY ENGINEER

1. Advise management and supervision staff on all contracts and workshops within
Saudi Aramco on their safety performances and enforce compliance with statutory
requirements and ensure full implementation of the GCSCC Safety Policy.
2. Check the adequacy of fire precautions and emergency procedures and ensure
corrective measures are implemented where needed.
3. Carry out accident investigations and make remedial recommendations. Maintain
any mandatory company, Government or Municipal records, reports and
notifications.
4. Report to CEO and Contracts Manager on all aspects of Company Safety. Promote
and participate in Safety Committee activities, as necessary.
5. Issue to sites a full set of safety documentation (manuals, registers, forms, posters,
etc.) and thereafter replenish the stock on request. Ensure the maintenance and
regular inspection of all site records.
6. To co-ordinate; with Health Department for health affairs.
7. Organize site safety training when deemed necessary in conjunction with the Project
Manager.
8. Instruct Subcontractors on safety procedures and company safety policy.
9. Liaise with Site Management at all levels.
10. Carry out inspection of sites, workshops, camps and offices at regular intervals.
Prepare reports and issue them to the responsible manager. Review the corrective
action.
11. Ensure that Safety Records and Statistics are compiled and reviewed annually.
12. Assisting Project Managers in preparation of Project Safety Plan land monitoring
regular updating thereafter.

7.3 CONTRACTS MANAGER

1. Ensure the effective implementation of the company safety policy and the safety
management system on all company projects and or workshops and offices in the
Kingdom of Saudi Arabia.
2. Ensure all employees; especially new recruits receive adequate safety training.
3. Apply the same standards of Health, Safety and Welfare to Sub Contractors and
other employed on projects under the control GCSCC.
4. Ensure that accidents and dangerous occurrences are properly investigated and
action taken to prevent recurrence.
5. By example, encourage good safety practices on sites and workshop.
6. Give support and encouragement to Department Managers, Project Managers,
Construction Managers, Project Engineers and Workshop Manager to enable them
to fulfill the duties and responsibilities imposed on them by GCSCC Safety policy
and current legislation.
7. Ensure that all safety correspondence, reports, etc. concerning contracts under the
supervision of Project Managers, Construction Managers and Project Engineers are
effectively circulated for information and necessary action.
8. Confirm the competence of his supervisory staff with regard to safety aspects.

7.4 DEPARTMENT HEADS

1. Ensure adequate investigation is carried out at the tendering, temporary works


design and planning stages to define potential hazards or processes and establish
suitable precautionary measures before work commences.
2. Responsible for making sure that the safety policy statement is observed both in the
office and when staffs are on assignment out of the office, especially for new staff.
3. Where staffs are unfamiliar with the operation of any equipment, training must be
given.
4. Staff should not expose themselves to any unnecessary risks in the execution of their
wok. Any hazards should be immediately brought to the attention of their manager
or enable corrective measures to be initiated.

7.5 PROJECT MANAGER/CONSTRUCTION MANAGER

1. Project and Construction Managers will establish a safety organization for their
specific site formalized by the issuing of a Project Safety Plan as part of the safety
management system.
2. Delegate as appropriate, the necessary duties and tasks and ensure that all members
of staff and operatives are competent to carry them out. Ensure the proper
certifications of equipment and users and those adequate records are kept.

3. Ensure that a permanent Site Safety Officer is appointed who has received adequate
safety training/experience to enable him to carry out the task.

4. Carry out an inspection procedure to ascertain that all activities are undertaken with
due regard to Saudi Aramco Loss Prevention Program.
5. Ensure that in the event of any accident or dangerous occurrence involving an
employee, the required reports are made out and submitted to the appropriate
Department.
6. Ensure that all accidents or dangerous occurrences are investigated thoroughly.
7. Arrange for all Foremen to be given precise working instructions covering their
responsibilities for correct working methods. In particular ensure that they do not
allow employees to take unnecessary risk.
8. Ensure that Subcontractors are working in a safe manner and are complying in all
respects with the statutes and regulations affecting their particular activities.
9. Cooperate in Company safety training activities and ensure the pre-arranged
attendance of employees.

7.6 SITE SFETY OFFICER

1. Ensure the company safety procedures are followed on site through the authority of
the Project manager.
2. Ensure that all safety statutory posters and notices are displayed together with the
Company Safety Rules and Regulations.
3. Ensure that all accidents and dangerous occurrences are reported and any necessary
remedial action is taken.
4. Ensure that Subcontractors are working in a safe manner and complying with Saudi
Aramco Loss Prevention Regulations.
5. Ensure that the location of medical facilities is made known to all employees.
6. Ensure that an adequate supply of PPE’s is available and; issued to employees and in
return, the employees are made aware of their responsibilities regarding the wearing
of these personal Protective Equipments.
7. Responsible for updating and keeping the site Accident Book.

7.7 FOREMEN AND SUPERVISORS

1. Ensure that all operatives are competent to carry out their respective tasks and those
required to operate plant have sufficient knowledge and training to enable the
operation to be carried out safely.
2. Ensure that safe systems of work are implemented and that sufficient information
and supervision is available and maintained for all operatives under their control.
3. Ensure that housekeeping is maintained.

4. Ensure that an adequate and safe means of access and egress is maintained
throughout each workplace.

5. Provide and; maintain adequate welfare facilities as required.

6. Ensure that, if delegated, all necessary forms and registers are maintained as
required

7.8 GCSCC EMPLOYEES

1. Be aware of the Safety policy and relevant procedures prior to commencing their
duties.
2. Use the correct tools and equipment for the job and use safety equipment and
protective clothing supplied, e.g. helmets goggles, ear protection, etc as instructed.
3. Keep tools in good condition and use them properly
4. Report to their supervisor any unsafe or unhealthy conditions and any problems; in
the site, equipment and accessories.
5. Develop a personal concern for safety for themselves and for others particularly new
employees and the general public.
6. Avoid improvisation which entails unnecessary risk.
7. Refrain from horseplay and the abuse of welfare facilities. Suggest ways of
eliminating hazards.
8. Not to operate any item of plant unless they have been specifically trained and are
authorized to do so.

8.0 SUB CONTRACTOR SAFETY RESPONSIBILITIES

8.1 OBJECTIVES
The objectives of this document are to ensure that the standards for safety are achieved and
that all works are undertaken safely. It is the duty of the subcontractor to be aware of and
work in accordance with current legislation and to cooperate with GCSCC on matters of
Safety and Loss Prevention.

8.2 Training and Competence of Employees and Notification of Hazards

Each Subcontractor must ensure that his employees are adequately trained and experienced to
carry out their work safely.

To this end the Subcontractor must also ensure that specific hazards likely to be experienced
on the site, whether notified to him or discovered by him, are notified to his workforce
together with any precautions to be taken and Saudi Aramco rules to be observed. Similarly,
such hazards should be notified to his subcontractors and to the Company workforce.

Subcontractors are legally responsible for their own workforce and for ensuring that their
work is carried out in a safe manner. Personal Protective Equipment (PPE)
will be issued by the Subcontractor to its workforce or failing that will e issued by GCSCC
and charged to the Subcontractor accordingly.

8.3 Site Supervision

Subcontractors must ensure that an employee of suitable seniority and authority is always
present on site during the course of subcontract works to supervise and direct those works
and to receive and implement instructions from GCSCC Project Manager.

9.0 WORKING HOURS

During the course of work, GCS shall be working 6 days a week, 10 hours a day.

Normally, the work shall be carried out during daylight hours. If during the course
of the project night work is required, all support facilities such as lighting shall be
provided in order to maintain safe working conditions.

It is presently anticipated that working hours will be from 6.00 am to 5.00 p.m with
one hour lunch break. However, these hours may change depending on the time of year.

10. SAFETY ORIENTATION

Each employee, upon his arrival on the Project, shall be instructed on the project’s
Safety Procedures and Requirements. The Site HSE Manager is responsible for the
implementation of the Safety Orientation Program. The Safety Orientation Program
shall be prepared by Site HSE Manager and will be conducted by qualified instructors.
This orientation is compulsory and all new employees will receive it before starting to
work at the construction site.

An orientation checklist shall be issued to each new arrival during their orientation.
This permanent record shall be held on file by HSE Manager.

The HSE Manager and Construction Manager shall ensure that individuals who are
required to attend Saudi Aramco certification trainings, attend such trainings promptly
and maintain their certification in an unto date manner. Individuals who carry such
certifications shall present their authorization pass to Saudi Aramco upon request
without fail.

11. SAFETY INSPECTIONS

GCSCC Company Safety Supervisor will visit sites and carry out site inspections.
Subcontractors must also co-operate in these inspections. Coordination of visits by
Subcontractors Safety Supervisor will be maintained through the Site Safety Office.
When Sub-Contractors own Safety Supervisor visit the site they must report their arrival
and departure to the Project Manager together with a written report of their findings.

11.1 Safety Inspection Procedure

The Company Safety Supervisor will visit work sites and carry out site
inspections to ascertain that all activities are undertaken with due regard to Saudi
Aramco safety regulations and the approved codes of practice and safety
procedures by Loss Prevention Department. All Subcontractors must cooperate in
these inspections. Items to be inspected includes but not limited to:-

a. Personal Protective Equipment or PPE


b. Excavations/ Earthworks
c. Material handling/storage
d. Accesses/Ladders/Stairs
e. Scaffolding
f. Lifting Equipment
g. Portable tools
h. Electrical Equipment
i. First Aid Equipment
j. Office/Canteen
k. Storage room
l. Toilet/Drying room
m. Compounds/Camps
n. Fuel Storage
o. Generators
p. Fire Fighting Equipment
q. House Keeping
r. Barricade

If during the course of the inspection, hazards or breaches of safety


regulations are noted, the following action will be taken:

A) Immediately stopped work if it is being done in a dangerous


manner that could harm anyone.

B) The Company Safety Supervisor will recommend alternate action


to avoid accidents.

12.0 SAFETY REPORTS AND RECORDS PROCEDURE

On visiting site with the intention of making a safety report, the Company Safety Supervisor
will inform the Project Manager of his arrival. He will meet the Site Safety Office, review the
weekly report file and conduct a joint safety inspection.

If hazards or breaches of health and safety regulations are noted, the following action will be
taken:-

1. If work is being undertaken in a dangerous manner, it will be immediately


stopped.
2. If the items noted can be rectified immediately, the mater need not be formally
recorded.
3. If the items cannot be rectified immediately, the details must be entered onto the
Company Safety Engineer’s report, Appendix C which will be distributed to the
Project Manager for action with a copy to the Site Safety Officer and Contracts
Manager.

13.0 INJURY REPORTING

The project Manager and Site Safety Officer must ensure that all personal injury accidents,
whether GCSCC or subcontractor staff, are recorded in the Accident Book .

Appendix – B

The Company Safety Supervisor will review the Accident Book during routine visit. In the
event of an employee sustaining a specified major injury due to an accident at work, the
following immediate action must be taken:-

1. Inform the Site Safety Supervisor who must accompany the patient to the
hospital.
2. Telephone the Company Safety Supervisor and Contracts Manager
3. Enter details in Accident Book, Appendix – B

14.0 CONTRACTOR CAMP SANTATION AND SAFETY

14.1 Potable Water

a. An adequate supply of potable water will be provided in all places of work

b. Provided disposable drinking cups at each drinking water station.

c. Portable containers used to dispense drinking water shall be capable of being


tightly closed and equipped with a tap. Water shall not be dipped from containers.
The drinking water supplied, during hot weather, will be cooled.

14.2 Toilets at construction job sites

a. Toilets will be provided for employees according to the following table:-

b. Every sanitary convenience shall be sufficiently ventilated with proper lighting

14.3 Washing Facilities

a. The Project Manager will provide adequate and suitable washing facilities in near
proximity to the work site.
b. Provide nail brushes and soap in case employees are working on coating and
painting jobs, herbicides, insecticides or other poisonous substances.

14.4 Shelter and Eating Place

The Project Manager will provide an area for taking meals and drinking water.

15.0 SAFE WORK PROCEDURES

15.1 PROTECTION FROM HAZARD

1) GCSCC will initiate and maintain such procedures as may be necessary to


comply with accident prevention.
2) Such procedures shall provide for frequent and regular inspection of the job
sites, materials and equipment, to be made by competent persons designated by
the senior management.
3) The use of any machinery, tool, material or equipment which is not in
compliance with any applicable requirement is prohibited. Such machinery, tool,
materials requirement shall either be identified as unsafe by tagging or licking
the controls to render them inoperable.

4) GCSCC shall instruct each employee in the recognition and avoidance of unsafe
conditions and the regulations applicable to his work environment to control or
eliminate any hazards or other exposure to illness or injury.
5) Employees required to handle or use hazardous substances shall be instructed
regarding the safe handling and use and be made aware of the potential hazards,
personal hygiene and personal protective measures required.
6) Employees required handling or using flammable liquids; gases or toxic
materials shall be instructed in the safe handling and use of these materials and
made aware of the specific requirements contained in other pats of this
document.
7) All employees enquired to enter into confined or enclosed spaces shall be
instructed as to the nature of the hazards involved, the necessary precautions to
be taken and in the use of protective and emergency equipment required.

16.0 ACCIDENT PREVENTION SIGNS AND TAGS

1. General

Signs and symbols shall be visible at all times when work is being performed, and shall
be removed or covered promptly when the hazards no longer exist. The colors and
symbols use shall conform to International standard. The text shall be in ENGLISH,
ARABIC AND ANOTHER LANGUAGE known to the majority of the workmen.

2. Danger/Prohibition signs shall be in red, meaning an action is forbidden or stopped. The


symbols shall be inserted inside the circle in black and the text in ENGLISH, ARABIC
AND ANOTHER LANGUAGE as white letters on a red base.

Warning signs shall be warn/advise of a hazard/danger in yellow and black. The black
outlined triangle shall contain a black symbol on yellow base.

Mandatory signs shall be in blue and white and shall be complied with by all persons.
The white symbols and text shall be located inside a solid blue circle.

The mandatory signs shall be used for giving a positive instruction.

Safe condition signs shall show the way to safety with white and text on a rectangular
green background.

Construction areas shall be posted with legible traffic signs at points of hazard. All traffic
control signs or devices used for protection of construction workmen shall conform to
Road Traffic Rules.

Accident prevention tags shall be used as temporary means of warning employees of an


existing hazard, such as defective tools, equipment etc. They shall not be used in place of
or as substitute for accident prevention methods.

3. Barricades shall be provided as follows:-

Barricade for protection of employees against any fall into different levels, opening or
warning against any dangers nearby.

Barricades for protection of employee’s ad public relating to street and highway traffic
diversions, excavation trenches and pits.

Saudi Aramco concerned departments shall be notified and permission will obtain before
carrying out any traffic diversion activities.

Barricades for protection of public against.

• Material falling from building under construction


• Building material interfering with the pedestrian and vehicular movement in public
streets/highway or public area.

The contractor shall arrange for perimeter fencing to protect the public, particularly
children from hazards. Caution boards to be provided.

4. Flagman

When operations are such that signs, signals and barricades do not provide the necessary
protection on or adjacent to a highway or street, flagman or other appropriate traffic
control shall be provided. Reflective jackets must be used by flagmen.

17.0 WORK PERMIT

During the execution of the work throughout the duration of the project work permit shall be
secured by Saudi Aramco certified work permit receiver each day or shift work depending on
the agreed conditions with the work permit issuer of Saudi Aramco.

17.1 FUNCTION OF WORK PERMIT

Work permit is important in maintaining a safe work environment and is a requirement for all
Saudi Aramco jobs in restricted areas. It authorizes a specific activity at a specific time and
place in Saudi Armco’s designated restricted area. It also enlists the hazard, and the safety
precautions that must

be taken. The work permit serves as an official written record of conditions and requirements
agreed upon by the issuer and the receiver.

17.2 TYPE OF WORK PERMIT

1) Release of Hazardous Liquids or Gases, Form 924-1 (Yellow color)


This is required when opening a line or removing pump that may emit or release
flammable and poisonous gases.
2) Hot Work Form 924- 2 (Red Color)
Used when a possible heat sparks or flame is present in an equipment to work on.

3) Cold Work, Form, 924-3 (Blue Color)


This form is for the work that will not produce sufficient energy to ignite such as
insulating and cleaning or sand removal and brush painting, etc.

4) Confined Space Entry, Form 924-4 (Green Color)


Required for working in a confined space which may have dangerous atmosphere
such as inside tanks/vessels or any excavations or 4 feet or deeper.

17.3 ISSUE AND RECEIPT OF WORK PERMITS

A work permit is only issued to an authorized recipient who is holding a valid work
permit receiver certificate issued by Saudi Aramco and must be obtained from the
operating supervisor of the plant or the restricted areas before the start of any work.
17.4 LOCATION OF THE WORK PERMIT

The permit must remain in the job site at all times and the receiver must keep a copy in
his possession or within view of the job site for the duration of the job, so that it may
be presented upon request.

In case the receiver leaves the site due to unavoidable circumstances, he shall, it the
issuer concurs, give the permit to the issuer or to a senior crew member to keep in safe
custody until he returns.

17.5 CANCELLATION OF WORK PERMIT

A breach in the conditions of issue, or change in circumstances, may lead to the


cancellation of a work permit by the operation staff who are fully authorized to do so.

If the work permit is withdrawn, all works will be stopped and all equipment be shut
down until a new work permit is issued.

17.6 CLOSING OF WORK PERMIT

All work permits shall be closed out by both the issuer and the receiver after job
completion or at the end of each shift. Exception shall be when the distance and
remoteness of the site make daily signing impractical, and; it is so stated when the
work permit is; issued.

17.6 RULES GOVERNING WORK PERMIT PROCEDURE

1) Issuer and receiver must inspect job site together before signing the work permit.
2) Issue the correct permits for the job: hot, cold, confined space entry, and/or gas
release.
Two or more permits may e required for a single job.
3) Issuer and receiver must both have in their possession a valid work permit
certification card, issuer and receiver respectively.
4) J-W Snuffer gas test and /or H2S gas test and /or oxygen analysis test must be made
before issuing work permit.
5) Job description and equipment used must be clearly stated on the work permit. Be
specific, issue permits for a single pump, drum, etc.
6) All tick boxes must be correctly filled in and gas readings indicated.
7) Proper lockouts, hold tags, and blinds must be used where applicable (multiple clips
with lick, and/or chains with padlocks)
8) Work permits should be issued for the specific period of time required to complete
the job.
9) To extend time work permit beyond one shift, the oncoming shift issuer must inspect
job site, write in extended time and sign permit.
10) Special precautions such as requirements for fire watch, Scott air packs, life lines,
barricades, etc.
11) The work permit must remain on the job site in a conspicuously visible place while
work is going on. If an emergency develops, the permit must be withdrawn
immediately and all work stopped without questions.
12) The work permit must be closed out after a jib completed. Issuer and receiver must
inspect the job site and sign off the work permit.

18 WELDING AND CUTTING EQUIPMENT PROCEDURES

18.1 Arc Welding

1). All arc welding and cutting cables shall be of the completely insulated, flexible type,
capable of handling the maximum current requirements of the work in progress.
2). Pipe-lines containing gases or flammable liquids or conduits containing electrical
circuits, shall not be used as a ground return.

3) All ground connections shall be inspected to ensure that they are mechanically strong
and electrically adequate for the required current.
4) When the arc welder or utter has occasion to leve his work or to stop work for any
appreciable length of time, or when the are welding or cutting machine is to be
moved, the power supply shall be switched off.
5). All arc welding and cutting operations shall be shielded by noncombustible or flame-
proof screens which will protect employees and other persons working in the
vicinity form the direct rays of the arc.

6) Adequate mechanical fume extraction, and forced ventilation shall be arranged


whenever work does not take place in the open air.
7) Employees performing any type of welding, cutting shall e protected by suitable eye,
face, hand, body ad respirator protective equipment.
8 No Welding cutting shall be done where the application of flammable paints or the
presence of the flammable compounds or heavy dust concentrations create a hazard.
9 Suitable fir extinguisher equipment shall be immediately available in the work area and
shall be maintained in a state of readiness for instant use.

18.2 Gas Welding and Cutting

1). Valve protection caps for compressed gas cylinders shall be in place and secured
during transportation, usage or storage.
2). When cylinders re hoisted, they shall be secured on a cradle, signboard, or pallet. They
shall not be hoisted or transported by means of magnets or chain or wire slings.
3). When cylinders are transported by powered vehicles, they shall be secured in a vertical
position.
4). Compressed gas cylinders shall be secured in an upright position.
5). Cylinders shall be moved in purpose built trolleys.
6). Cylinders Shall be placed where they cannot become part of an electrical circuit. Elect
odes shall not be struck against a cylinder to strike an are.
7). Cylinders containing oxygen or acetylene shall not be taken into confined spaces.
8). Hoses shall be kept for one type of gas only and easily distinguishable from each
other.
9). Pressure regulators including their gauges shall be in proper working order while in
used.
10). Cylinders, cylinder caps and calves, couplings, regulators, hose and apparatus shall be
kept free from oil or greasy substances and shall not be handled with oil hands or
gloves.

19 PERSONAL PROTECTIVE EQUIPMENTS (PPE)

Only approved type of PPE and the requirement of to e worn at all time while in the job
site.

19.1 Head Protection


Employees working in areas where there is possible danger of head injury from impact,
or form falling or flying objects, or from electrical shock and burns, shall be protected by
protective helmets. This includes all building construction and demolition sites.

Color code for GCSCC safety helmets will be as follows:-


Supervisory and Senior staff including visitors - White
All labors, tradesmen working for civil - Yellow
All Rigger - Green
Electrician - Gray
All others - Blue

19.2 Hearing Protection

Wherever it is not feasible to reduce the noise levels or duration of exposures to


acceptable levels, ear protection devices shall be provided and used.
Ear protection devices inserted in the ear or earmuff shall conform to International
Standards and shall be fitted individually.

19.3 Eye and Face Protection


Employees will be provided with eye and face protection equipment when machines or
operations present potential eye or face injury from physical, chemical or radiation
agents.
Safety Glass with side shield (Z-87) will use at all the time.

19.4 Protective Clothing


All GCSCC employees will have adequate and suitable clothing to protect from hazards
such as liquids and vapors, excessive heat and cold, electricity, impacts, ionizing radiation
and also general weather conditions.

19.5 Hand Protection


All GCSCC employees will have adequate and suitable hand protection for handling
rough, abrasive, sharp or excessively coarse material such as reinforcement rods, precise
concrete etc, toxic and corrosive materials, electricity, and hot materials.

19.6 Foot Protection


All employees are required to wear suitable and adequate safety shoes according to their
duties and working environments. The safety shoes should be strong enough to withstand
impact and sole penetration expected on a construction site.

19.7 Fall Protection


Falls are one of the major safety concerns because they result in the most serious injuries
and account for the majority of fatalities in the construction industry. A full body harness

shall be worn when there is a potential of a fall and where no other fall protection is
provided.
All employees and Subcontract personnel who are required to work on or inspection of
items higher than 6 feet (1.8) meters) shall were and use a full body harness.

20 TOOLS AND PORTABLE POWER TOOLS

20.1 General
1). All hand and power tools similar equipment used at the construction site, shall be
maintained in safe condition.
2). When power operated tools are designed to accommodate guards, they shall be
equipped with such guards when in use.
3). Belts, gars, shafts, pulleys, sprockets, spindles, drums fly wheels, chains and other
reciprocating, rotating or moving parts of equipment shall be guarded if such parts
are exposed to contact by employees or otherwise create a hazard.
4). Employees using hand and power tolls and exposed to the hazard of falling, flying
abrasive and splashing objects, or exposed to harmful dusts, fumes, mists, vapors
and gases shall be provided with the appropriate personal protective equipment
necessary to protect them from the hazard.

20.2 Hand Tools

1). GCSCC shall not issue permit for the use of unsafe hand tools.
2). Wrenches, including adjustable pipe end and socket wrenches shall not be used
when jaws are sprung to the point that slippage occurs.
3). Impact tools such as drift pins, wedges, and chisels, shall be kept free of
mushroomed heads.
4). The wooden handles of tools shall be kept free of splinters or cracks and shall be
kept tight in the tool.

20.3 Power Operated Hand Tools

a. Electric Power Operated Tools

1). Electric power operated tools shall either be of the approved double insulated
type or grounded.
2). The use of electric chords for hoisting or lowering tools shall not be permitted.

b. Pneumatic Power Tools

1). Pneumatic power tools shall be secured to the hose by some positive means to
prevent the tool from becoming accidentally disconnected.

2). Compressed air shall not be used for cleaning purposes unless effective
measures have bee taken against flying particles.
3). The manufacturer’s safe operating pressure for hoses, pipes, valves, filters,
and other fittings shall not be exceeded.
4). The use of hoses for hoisting or lowering shall not be permitted.

c. Hydraulic Power Tools

The fluid used in hydraulic powered tools shall be fie –resistant and shall retain its
operating characteristics at the most extreme temperatures to which it will be
exposed.

21.0 LADDER PROCEDURES

1. No ladder standing on a base shall be used unless


• It is securely fixed near to its upper resting place or in the case of a vertical ladder,
near to its upper end:
• It has a level and firm footing and is not standing on loose bricks.
• It is secured to prevent undue swaying or sagging and
• It is equally and properly supported on each stile or side.

2. If a ladder, standing on the ground cannot be fixed to prevent slipping, then someone
shall hold it at the base when it is being used.
3. No ladder shall be used unless

• It extends to a height of at least 1.0meter above the place of landing or the highest
rung to be reached: or
• There is other hand hold; and
• There is sufficient space at each rung to provide adequate foot hold.

4. No ladder shall be used in a horizontal position as platforms, runways or scaffolds.


Portable metal ladders shall not be used for electrical work or where they may contact
electrical conductors.

5. No folding step-ladder shall be used unless; it has a level and firm footing.

6. Where a ladder passes through an opening in the floor of landing place, the opening shall
be as small as is reasonably practicable.

22. CARTRIDAGE OPERATED TOOLS

22.1 GENERAL:

GCSCC shall ensure that only properly trained personnel will handle this type of tool and
shall undergo training period duly certified by tool vendor. Use of cartridge operated
tools will be in accordance with Saudi Aramco G 12.100, ANSI A 10.3- 1985 and he
manufacturer’s specifications shall be subject to review and concurrence by the Loss
Prevention Department.

22.2 STORAGE

Safe and secure storage for cartridge and tools must be provided on the job site. Storage
must be fire proof, dry, must be capable of being locked and should be sited in a clear,
but easily accessible location where constant supervision can be maintained.

a. No person shall use the cartridge operated tools unless he has been trained and
experienced in the operation.
b. The supervisor shall ensure that the employees are made aware of the hazards and
the manufacture’s operating instructions.
c. The supervisor shall ensure the proper storage, issue and use of cartridges.
d. The operator shall use personal protection equipment (PPE) when using cartridge
operated tools.
e. Every cartridge operated tool, where thee is potential hazard of flying pins and
splinters, shall be fitted with guards of sufficient size and shall be firmly held with
the surface.

23.0 ELECTRICAL INSTALLATIOPN AND EQUIPMENT

1. On the award of a contract, the Project Manager will liaise with Aramco proponent for
provision of Electrical supply.

2. Inform Saudi Aramco on the system required for temporary use.


3. Arrange for temporary power in the interim.
4. Only properly grounded 110 V weather tight convenience outlets will be installed.
5. Every temporary electrical installation will be under the control of a competent person
say, Electrical Engineer who will accept responsibility for this safety and use.
6. Each installation must be regularly inspected and tested to ensure that wiring;
connections, breakers, switches, plugs etc. are being maintained and used in a safe and
efficient manner.
7. Circuits found to have faults liable to lead to a dangerous situation will be immediately
disconnected from the supply until such faults have been properly corrected. Project
Manager and Electrical Engineer will be informed accordingly.
8. All electrical equipment and cables shall follow the National Electrical Code and special
precautions shall be taken in view of rough conditions on site.
9. The lengthy cables for supplying movable plant and portable tools shall be protected
from damage. The tools shall be disconnected before any adjustments are made or
attachments changed.
10. Supply cables for fixed plant shall be routed clear of construction work and be protected
against traffic movement.

23.1 ELECTRICAL TOOLS AND LIGHTING

1. Electrical tools and extension chords shall be inspected each time they are issued and
returned.
2. Only properly grounded 110 V weather tight convenience outlets will be installed.
3. Never use exposed wires on portables.
4 Ground fault circuit interrupters of GFCI shall be used in the construction site.
5. Boxes of main switches, fuses, circuit breakers, etc shall not be left open after repairs are
made
6. Use only the approved insulated or grounded electrical tool.
7. Electricians must be qualified and shall wear the necessary Personal protective
Equipment prior to work.
8. A competent person will inspect all electrical tools monthly basis and color code all the
tools.

23.2 UNDERGROUND CABLES

1. No mechanical excavations shall be started before test trenches have been dug manually
and the cable location is established.
2. When underground cables are exposed, they shall be protected as necessary to avoid any
damage. But in case of damaged cable, immediately notify Saudi Aramco representative
and barricade the area.
3. When multiple cables exit in an excavation, the cable to be worked on shall be identified
by electrical means unless its identity is obvious by reason of distinctive appearance.

24.0 SCAFFOLDING

The safe and efficient erection, use dismantling and storage of scaffold and elevated work
platforms are considered an important objective in maintaining a safe work environment.

All employees are charged with the responsibility to provide full support and act in strict
accordance with the provisions of this procedure. Every employee who observes violations of
these requirements has the responsibility of reporting unsafe conditions or unsafe practices to
supervision immediately, and shares with supervision the responsibility for following safety
guidelines. All scaffolds shall be constructed as per the requirements of Saudi Aramco GI
8.001 & Saudi Aramco Scaffold Handbook.

Procedure

The minimum requirements of this procedure must be reviewed thoroughly by persons


responsible for implementing, enforcing, administering, and monitoring the erection, use,
dismantling and storage of scaffolds and elevated work platforms, following GI 8.001 and
Saudi Aramco Scaffold Safety Handbook.

All scaffold Supervisor and inspectors shall be Saudi Aramco Certified.

General requirements

1. Any elevated work presents a potential fall hazard, it is essential that precautionary
measures be thorough.
2. No scaffold shall be erected, moved, dismantled or altered except under the supervision
of the Scaffolding Supervisor.
3. Guardrails and toe-boards shall be installed on all open sides and ends of platforms more
than 6ft above the ground or floor.
4. All workers shall tie off with a full body harness when there is no handrail or guardrail,
when there are openings over 18 in. in the working platform, or when on suspended
working platforms. Harness and tie off practices apply to all travel by workers. If
necessary vertical drop lines shall be used to attach lanyard. Scaffold will be tagged with
Yellow tag Full body harness required.

Color Codes

• GREEN Scaffold safe to use, subject to load limits on the tag.


• YELLOW Scaffold structurally sound but handrails or other parts missing. The
use of a full body harness will be required for “Yellow” tagged
scaffolds.
• RED DOES NOT USE SCAFFOLD. Only Authorized Scaffolder working
on the scaffold may access it.

Within a period not exceeding 7 days, the scaffold will be re-inspected by a Scaffolding
Supervisor and a signature and date placed upon the tag.

25.0 CRANES AND RIGGING EQUIPMENTS

25.1 STABILITY OF CRANE OR ANY OTHER LIFTING EQUIPMENTS


1. Appropriate precautions shall be taken to ensure the stability of crane used on
soft or uneven surfaces or on slop.
2. Cranes shall be anchored to the ground or other foundations but rail tracks
shall not be used as anchorages.
3. Cranes shall be anchored to the ground or other foundations b rail track shall
not be used as anchorages.
4. A diagram of the counterweight or ballast indicating the position and weights
shall be affixed on the crane.
5. Anchorage and ballast materials shall be examined by a competent person on
each occasion before the crane is erected.
6. Whenever a crane is erected or resisted or any alterations to the anchorages or
b ballast are made, the anchoring and ballasting shall be examined, before that
crane is put to work and tested by applying a load 25% higher than the
maximum rated load. A report of this test s shall be maintained at the
construction site.
7. No crane shall be used or erected under weather conditions likely to endanger
its stability. After such weather conditions, the anchorage and ballast materials
shall be examined before the crane is used.

25.2 RESTRICTION ON THE USE OF CRANES:

1. The hoisting mechanism of a crane shall not be used for any purpose other
than raising or lowering a load vertically unless the operation is under the
supervision of a person competent to ensure that it can be done safely and
without damage to the crane.
2. Crane jibs shall not be worked at a radius greater than specified on the test
certificate.

25.3 RIGGING EQUIPMENT

1. No chain, rope or lifting gear shall be used in raising or lowering r as a means


of suspension unless it is of good construction, sound material, adequate
strength, suitable quality and free from patent defects.
2. It has been tested and examined by a competent person and obtained a
certificate (signed by the person0 specifying the safe working load and
reviewed by Loss prevention Department.

3. Rigging equipment for material handling shall be inspected by CCFIU prior to


use on shift and as necessary during its use to ensure that it is safe. Defective
rigging equipment shall be removed from service.
4. Rigging equipment shall not be loaded in excess of its recommended safe
working load.
5. In case of a multi sling, the safe working loads at different angles of the legs
shall not be exceeded. The upper ends of sling legs shall be connected by
means of a shackle ring or link of adequate strength.
6. Rigging equipment, when not in use, shall be removed from the immediate
work area so as not to present, a hazard to employees.
7. Slings shall not be shortened with knots or bolts or other makes shift devices.
8. Every sling used for raising or lowering on lifting appliance shall be securely
attached to the appliance, and the method of attachment shall not be a method
likely to result in damage to any part of the sling, rope or to any lifting gear
supporting it.

26.0 MECHANICAL EQUIPMENT

26.1 General Requirements:-


1. Before any mechanical equipment is used in the site, a corresponding work permit
must be obtained first.
2. All machinery shall be inspected before being placed in service and at regular
intervals thereafter.
3. Maintenance schedules shall be established for each mechanical equipment and
no repair, adjustment, or replacement of parts on moving machinery is permitted.

26.2 WOOD WORKING MACHINE

1. The cutters of every wood working machine shall be enclosed y a guard of


substantial construction.
2. No person shall make any adjustment to any guard on wood working machine
while the cutters are in motion.
3. At all times while the cutters are in motion, the guards and devices shall be
kept constantly in position and properly secured.
4. Every wood-working machine shall be provided with a start and stop controls
which can be operated quickly and easily b the men using the machines.
5. The space around all wood working machines shall be clear and unobstructed
without risk of injury to persons employed.
6. Adequate training will be given to the operator.

26.3 CIRCULAR SAW MACHINES

1. The part of the saw blade of every circular sawing machine which is below the
machine table shall be guarded.

2. Every circular sawing machine shall be provided with a riving knife fixed
below the machine table, behind and in direct line with the saw blade.

26.4 GRINDING MACHINES

1. Every abrasive wheel shall be mounted properly.


2. The safe working peripheral speed of the grinding wheel shall not be
exceeded.
3. A guard shall be provided and kept in position at every abrasive wheel in
motion.
4. Where a work pieced rest can be provided, it shall
• Properly secured.
• Adjusted as close as practicable to the wheel
• Of substantial construction and properly maintained.
5. Adequate PPEs especially eye protection shall be provided and used by
workmen.
6. Adequate training is needed for the operatives.

26.5 FORKLIFTS TRUCKS

1. It is essential that operators must have a valid Saudi Arab Government heavy
equipment license and Saudi Aramco certificate.
2. Especial equipment fitted to the truck, in addition to or in place of fork shall
be designed for the specific machine.
3. The truck shall be quipped with overhead protection.
4. When traveling with a load on the forks, the forks should be as low as possible
to maintain stability.
5. If the load being carried obstructs the operator’s view, he should travel in
reverse.

26.6 DUMPTRUCKS

1. Dump trucks shall be properly maintained and cleaned at regular intervals.


2. The latch on dumper skips must be in good working order, and the release
mechanism should function smoothly.
3. Dumpers are not designed to carry passengers. It must be strictly forbidden for
employees to ride in the skip or in te engine cover.
4. When repairs or maintenance are being carried out on a hydraulically operated
dump truck, the dump body should be fully lowered. If it is necessary to have

it in raised position, it must be blocked. Do not rely on the hydraulic ram to


support the raised body for an extended period.

27.0 TRANSPORTATION

27.1 Driver Requirements:-


Only person authorized by their supervisor and in possession of a current and valid
Saudi Arab Government driving license are allowed to operate Company
transportation.

27.2 Driver’s Responsibilities:-

1. If you drive a vehicle on site, or the public highway, drive with care and give
consideration to your workmates and the public. As a driver, you must report
immediately to your foremen. The condition of any vehicle under your control if you
feel there is something mechanically wrong, i.e. brakes, steering etc.
2. Vehicles on sites must be in good condition and are not to b used in an improper
manner. Do not overload the vehicle in such a manner that it will interfere with the
safe driving operation.
3. No passenger is allowed to ride on any vehicle unless a passenger; seat has been
provided specifically for that purpose.
4. No person must remain in the cab of a vehicle when being loaded by a lifting
appliance.
5. Vehicles used for transit of men must have fixed seats, never; sit on the sides of
vehicles.
6. When reversing, do not rely on mirrors, always obtain assistance and reverse under
the direction of an attendant:-
7. Never leave a vehicle unattended with the engine running.
8. The applicable speed limits must ALWAS be observed.

27.3 DESERT DRIVING

When traveling in the desert, ensure that:

2. You know where you are going and your immediate supervisor knows your
destination and route.
3. Your vehicle is in good condition with adequate fuel, oil and water.
4. Your car complies with all legal paper requirements and you are in
possession of current and valid driving license.
5. Make sure your vehicle has tools, equipment and spares for emergency use.
6. You have sufficient food and drinking water to sustain you until rescue,
should you be stranded.

7. Bring along with you your mobile, radio or any means of communication as
the person lost or stranded in the desert must stay with the vehicle as it
provides shelter and to make it easier for searchers to locate him.

27.4 TRAFFIC SAFETY

In plant driving will be kept to a minimum and any unsafe driving practices observed
shall be subject to disciplinary action.

Designated drivers shall have a valid Saudi Arabian driving license. Each diver shall
be instructed on SAG, Saudi Aramco and Plant traffic regulations. Unauthorized
operation of vehicles (construction or private) will result; in immediate termination.

Aramco Traffic Regulations and Saudi Arabian Traffic Law shall be adhered to at all
times. Safety belts for driver and passengers shall be worn at all times except ion the
case of buses where seat belts are mandatory only for the driver. At no time will
passengers be transported in the back of pick-up trucks or on heavy equipment.

The towing of small equipment such as compressors, welding machines, etc. is


allowed, but only after the equipment is properly and safely connected to the tow
vehicle (i.e. all safety pins, safety chains shackles and brae ;in place). The towing
speed of the vehicle must not exceed 40 KM per hour at any time.

Subcontractor shall comply with all pertinent requirements of the Saudi traffic
regulations for transporting construction materials and supplies between storehouses
and WORK Site (See G>I Nos 1183.215 and 6.030 and section 1-13 of the Saudi
Aramco Construction Safety Manual)

The traffic plan inside the site area will be prepared and updated monthly to have
proper logistic control after commencement of site activities, in accordance with site
situation.

28.0 INJURY AND DAMAGE REPORTING

When any incident or dangerous occurrence happens in the work site, GCSCC will notify the
proper authorities detailed below:-

These notifications of accidents are in addition to the requirements for all other occurrences.

The Project Manager shall maintain an accident book containing details of accidents, name of
the injured, date of accident, date of return to work, the number of days the injured person
was away from work, brief description of accident and this shall be made available in the site
office for inspection.

29.0 EXCAVATION/ SHORING

1. Prior to opening an excavation, effort shall be made to identify the presence of


underground installation; i.e sewer telephone water fuel electric lines etc. and
necessary precautions shall be taken. Prior permission thru work permit must be
obtained from Saudi Aramco.
2. The walls and faces of all excavations in which employees are exposed to danger
form a fall or dislodgement of earth, rock or other material shall be guarded by
shoring system, sloping of the ground or some other equivalent means.
3. The determination of the angle of repose and design of the supporting system shall be
based on evaluation of factors such as depth of cut, soil, loading, vibrations, traffic
and other sources.
4. Timbering or other support for any part of an excavations shall be of good quality
material
Inspected before use by Saudi Aramco
5. Every accessible part of an excavation, near to which employed persons are working,
shall be provided with suitable barrier.
6. Banks more than 1.5 meters high or trenches less than 1.5 meters in depth needing
protection due to condition of ground shall be shored, laid back to a stable slope or
some other equivalent means of protection.
7. When employees are required to be in trenches 1.2 meters deep or more, an adequate
means of exit, such as ladder or steps shall be provided and located so as to require no
more than 8 meters of lateral travel.
8. Bracing or shoring of trenches shall be carried along with the excavation.
9. Portable trench boxes of sliding trench shields may be used for the protection of
personnel in lieu of a shoring system or sloping.
10. Where any vehicle is used for tapping material into any excavation or pit or over the
edge of any embankment or earthwork, adequate measure such as stop logs,

barricades shall be provided to prevent such vehicle from getting within an unstable
distance of the edge. A banks man must accompany the vehicle.

30.0 CONFINED SPACE ENTRY PROCEDURE

This procedure shall be applied to Project work performed in enclosed or confined areas,
such
Tanks, vaults, bins ducts, sewers, tunnels, pipelines, and similar work place usually entered
through a manhole or other constricted opening. Confined space entry operations shall
always be preplanned. Before entering a confined space, a formal written entry procedure
shall be followed. It shall always be assumed that a hazard is present and the environment
shall be measured for contaminants before entry. Detection instruments and protective
equipment shall be obtained and kept readily available and collaborated for use.

All gas testing, conducted independent of permit, is performed by the TR Safety Group. All
Confined Space Entry Procedure shall be reviewed and approved by TR prior to beginning
the work.

All gas testing instruments being used shall be explosion proof and particulars etc be
reviewed and approved by the SHES Manager before putting them in service.

Breathing air compressors, if required for entering confined space, must be certified at least
every 6 months by third party.
Standby man, required by confined space permits are specially trained and authorized by the
TR Safety Department. These persons complete a special training course on duties,
responsibilities and procedures.

A consolidated listing of all authorized confined space watchmen shall be maintained by our
Safety Group. This standby man is an important safety requirement and only TR certified
standby men are used for the activity.

30.1 Procedure

1. Prior to assigning employees to enter a confined or enclosed space, the


responsible supervisor shall ascertain that the necessary air testing, protective
clothing, respirator equipment, emergency standby equipment, fire prevention
precautions, and personnel have been specified and provided.
2. The responsible supervisor shall be accountable for the posting of the Saudi
Aramco Confined Space entry Permit at the work location.
3. Personnel assigned to work in confined or enclosed spaces shall receive training
and instructions as to the nature of the hazards involved, the applicable safety
procedures, and the use of the required protective emergency equipment.

4. A quick means of safe access and egress, to the fullest extent possible and
practical, shall be available and maintained at all times during occupancy of a
confined or enclosed space.

30.2 Protective Equipment

1. All persons working in a confined space shall wear a full body harness
continuously from entrance time to exit time. The full body harness shall be
attached to a ½ in. manila lifeline, or equivalent, leading out of the exit. If the
work conditions warrant an exception of this requirement, this shall be approved
by the Safety Officer.
2. A standby attendant shall be assigned to work at the entrance to the confined area
to monitor the safety aspects of the job. This coworker shall be selected for his
capabilities in this type of assignment and for handling possible emergency
situations which could arise. He shall remain at his post whenever the work area
is occupied and maintain constant communication with the occupants. An
approved telephone or radio system shall be kept by the sand by attendant, to
communicate with Safety/Security Manager and/or Safety Office in case of
emergency. The standby attendant shall be available and may have to qualify in
the use of a self-contained breathing apparatus SCBA.

31.0 FIRE PREVENTION

31.1 GENERAL REQUIREMENT

1) The Project Manager and Company Safety Supervisor shall be responsible for the
development of a fire protection program to be followed throughout all phases of
the construction work, and shall provide appropriate fire fighting equipment as
prescribed by the Loss Prevention Department.

2) Access to all available fire fighting equipment shall be maintained at all times.

3) All fire fighting equipment shall be periodically inspected and maintained in an


operating condition. The Company Safety Supervisor will arrange to provide
adequate fire fighting training to the employees.

4) An adequate means of escape shall be maintained. Gangways, doors, and ladders


shall be kept clear at all times. In view of the changing nature of construction
operations, escape routes shall be frequently surveyed.

31.2 WATER SUPPLY

Where required, a temporary or permanent water supply of sufficient volume, duration,


and pressure, that will be supplied for fire fighting.

31.3 PORTABLE FIRE FIGHTING EQUIPMENT

1) Fire extinguishers shall be provided on each floor. In multistory building, at least


one fire extinguisher shall be located adjacent to stairways.

2) Travel distance from any point of the protected area to the nearest fire
extinguisher shall not exceed 100 feet (30 meters).

32.0 FORM WORKS

1. Form work shall be designed, erected, supported, braced, and maintained as per Saudi
Aramco approved standards so that it will safely support all vertical and lateral loads that
may be imposed upon it during placement of concrete. As far as practicable, steel units
shall be used.
2. Personnel shall not be allowed under or in close proximity of the form work during pour
operations.
3. Personnel not engaged in the pour operation shall stay of the pour area. A clear area shall
be maintained at 1-1/2 times the highest point of the form work.
4. Where the work requires a timber supporting structure, the timber shall be of suitable
quality and of adequate strength. Contractor shall obtain prior written approval from
Saudi Aramco before any timber supporting structure is erected.

33.0 FIRST AID AND MEDICAL CARE

1. The first aid facility shall be in accordance with Saudi Labor and workmen Law’s
Articles 134 & 1345 and staffed by a licensed nurse. A first aid log of injuries will be
maintained (Reff. to Attachment- First Aid Log). The Curriculum Vitae of the
licensed nurse will be submitted to TR for review.

2. The first aid facility along with an equipped ambulance shall be available at the work
site during working hours.

3. The responsibilities of first aid and medical personnel shall be to:


• Treat occupational injuries and illnesses
• Contribute to accident reports.
• Keep medical records, first aid log and the medical files
• Render re-treatment when required.

4. The fully equipped (per the requirements listed in Saudi Aramco Construction Safety
Manual, 4th Edition, Section 1-8 and GI 150.002 ) and dedicated ambulance, with
dedicated ambulance drier (s) shall be marked accordingly and maintained on standby
during working hours. The location of the nearest medical facilities location map
shall be kept in the ambulance.

5. First aid facilities shall be provided at the Contractor’s office, Subcontractor’s camps,
Lay down Area. Each facility will have a licensed First Aider with first aid kit.

34.0 HOUSE KEEPING PROCEDURE

This procedure is designed to give Supervisors and employees guidance in maintaining a neat
work area which should encourage safe work habits. Each craft will be responsible for
cleaning up their work is. No job is complete until the area has been cleaned up.

General

Housekeeping plays an important role in assessing Contractor safety performance on the


project. It is included as a separate section in the quarterly audits conducted by the Owner
and it is also an

incentive award category of the Safety Incentive Plan. Special attention must be given to
housekeeping.

During the course of construction, all debris and scrap material shall be kept away from the
work area. Work areas shall be cleaned at the end of each shift. Containers shall be provided
by Contractor for the collection and separation of waste, trash oily and used rags and other
refuse.

Garbage and other waste shall be disposed of at frequent and regular intervals in a manner
approved by Saudi Aramco.

GCS shall notify TR of any hazardous waste it will generate during performance of the
Work. GCS has the direct responsibility of maintaining proper storage of these wastes while
on site and will verify to TR I writing that the wastes have been disposed of in a legal manner
in accordance with the requirements of the Saudi Aramco.

GCS shall clear all combustible debris to a solid waste disposal site properly licensed by the
Saudi Aramco. “No open burning of debris or rubbish will be permitted at the project
jobsite”

Materials and supplies shall be stored in locations, which will not block access-ways, and
arranged to permit easy cleaning of the area. In areas where equipment might drip oil or
cause other damage to the floor surface, a protective cover of heavy gauge, flame resistant

oil-proof sheeting shall be provided between the equipment and the floor surface sheeting so
that no oil or grease contacts the concrete. This requirement is applicable to both finished and
unfinished floors.

All hoses, cables, extension cords, and similar materials shall be located, arranged and
grouped so that they will not block any access-way and will permit easy cleaning and
maintenance.

Orderliness

• Keep tools and working materials in proper containers.


• Store trash, waste, and scrap in proper containers.
• Store materials safely.
• Put cigarette stubs in butt cans.
• Keep small items in boxes or bins.
• Keep the floor clear of tools, rod ends, and metal shavings.
• Keep walkways clear.
• Ensure that work tables are occupied only by work at hand and the tools required for
the work being done.
• Do not leave aerosol cans on fabrication tables.
• Store or contain material so that fire has no place to start.
• Clean up tools and work areas as your job progresses.
• Keep cords and hoses 2 meters overhead or lay them flat out of walkways.
• Keep all material, tools and equipment in a stable position (tied, stacked or choked) to
prevent rolling or falling.
• Maintain clear access to all work areas.
• Keep stairways clear or debris.
• Clean up liquid spills right away.
• Keep barricades in order and good state of repair.
• Never throw tools, material or scrap below when cleaning at an elevated level of
platform.
• Always maintain good lighting (illumination) of the work areas.
• Do not block any emergency equipment or electric disconnect switch.
• Stack, store, or spot material so that it can be reached readily by workers and
materials handling equipment.

Inspection of job tools is part of good housekeeping. Worn or broken tools must be discarded
immediately.

NOTE

In general, at trash, waste, and scrap must be placed in properly placed trash cans and routes
leading to and from all work locations must be free and clear of obstructions and well
lighted.

35.0 JOB SITE INSPECTION

In order to comply with the Safety and Loss Prevention Program effectively, a frequent and
regular job site inspection is a must.

1. The use of any machinery, tool, material or equipment which is not in compliance with
any applicable requirement is prohibited. Such machinery, tool, materials requirement
shall either be identified as unsafe by gagging or locking the controls to render them
inoperable.
2. Barricades shall be inspected and make sure that it is installed for protection of employees
against any fall into different levels, opening or waning against any dangers nearby.
3. Those employees that are required to handle or use flammable liquids, gases or toxic
materials shall be inspected with the safe handling and use of these materials and must be
made aware of the specific requirements herein.
4. Check that no person other than the trained operator shll operates the heavy equipment.
5. Signs and symbols shall be visible in the site at all times or when work is being performed,
and shall be removed or covered promptly when the hazards no longer exist.

36.0 SAFETY TRAINING REQUIREMENTS

Apart from the need to meet statutory requirements, it is the company policy that all levels of
employees are given health and safety training by the appropriate level of management.
Subcontractors must also ensure that his employees are adequately trained and experienced to
carry out their work safely.

36.1Hand tools
• Always use the correct tools for the task and do not improvise.
• Check regularly if it is worn or damaged so it can be replaced.

36.2Fire Prevention

• Obey the No Smoking signs


• Know the location of fire exits and fire alarms.
• Keep solvents and other flammable/combustible materials in approved, properly
labeled containers.

36.3Personal Protective Equipment (PPE)

Always wear water proofs, rubber boots, helmets, goggles and ear protection while at work.
In addition to this, it is the duty of the company to make available a Safety Training Course
to all workmen at least once a month to keep them informed about the code of practice for
safe working methods.

37.0 WRITTEN GENERAL SAFETY RULES

The responsibility for safety at work rests upon all sectors of GCS Management and is not a
secondary issue. Project and Department Managers are personally responsible for safety and
welfare matters within their area of activity by applying the following safety rules and
regulations;-

1. Safe systems of work including the preparation of method statements for potentially
hazardous activities requiring official approval prior to commencement. This can be done
thru Work Permit System by Saudi Aramco.

2. Adequate instruction and information regarding the work and its corresponding hazards.

3. Effective control of hazardous substance and the provision of any necessary protective
clothing.

4. Effective fire prevention and fire control procedures.

5 Use of personal Protective Equipment (PPE) always while at work or as the situation
permit.

6 Maintain good housekeeping

7 Use of proper tools for the right job.

In parallel to the above, all GCS employees shall comply fully with this safety rules a both
the selection stage and throughout the execution of the work on site.

38.0 EMERGENCY PROCEDURES

In case of emergency or on hearing the “Alarm”, every supervisor shall ensure the following

5. No one is permitted to return to work until notification has been received from operations
or from the company representative that it; is safe to do so.
Contractor’s workforce and personnel are advised to join and take part in the “Drills”
conducted by the proponent to be aware of the current procedure being adopted in case of
real emergency situations.

39.0 CONTACTS AFTER OFFICE HOURS

The Contractor’s supervisor shall give his home address and telephone number (and that of
his replacement) to the company representative so that he may be contacted after working
hours in case of emergency involving the contractor’s job or equipment. These telephone
numbers are to be kept current and posted at the job site.

40.0 HELP IN CASE OF EMERGENCY

In the event of an emergency situation (serious personal injury, fire, critical damage to
operating equipment, etc) help may be obtained by contacting the nearest Saudi Aramco
Main Gate or Security Control Center. This may be done;

1) By telephone: Dial the emergency telephone number 110.


2) By radio: Radio an operations group that has a telephone in the Saudi Aramco system
and ask them to dial 110 when the main Gate or Security Control Centre cannot be
contacted directly.
3) By mobile phone if available.
4) By messenger: Send a messenger to the nearest telephone, radio or Saudi Aramco
gates.

When transmitting a message by telephone, radio, mobile or messenger, ensure that you
clearly identify yourself by giving

• Your exact location


• Nature of emergency
• Service required and repeat the message
• Your name
• Your badge number
Stay on the telephone until you are told to hang up. If possible, post a look out to direct the
ambulance, fire truck or helicopter to the right location.

41.0 MATERIAL AND WASTE DISPOSAL PROCEDURE

1. It is the duty of each Project Manager in charge of a site which provides, carries or
disposes of waste, to prevent the escape of waste from his control from excavation and
loading to final disposal. Waste disposal will be carried out as per Saudi Aramco
directions to a designated Sanitary Landfill.
2. The treatment of waste such as asbestos will be carried out in the strictly controlled
conditions as per Saudi Aramco standards and specifications.
3. Debris shall not be thrown from upper storey, but be removed either by machinery or
enclosed tilted passage/enclosed chutes. Accumulated debris shall be surrounded by
railing or wall.
4. All solvent waste, oily rags and flammable liquids shall be kept in fire resistant/metal
containers with lid until removed from work site.

42.O DEMOBILIZATION PLAN

Demobilization job must be carried out in a safe and orderly manner and in accordance with
the established safety policy by Saudi Aramco Loss Prevention Department.

1. Materials

Excess construction materials shall be kept in a suitable place and shall be segregated
according to their type. Used/rejected materials shall be disposed of at Sanitary Landfill.

2. Equipments

Heavy equipments shall be maintained and properly lubricated before keeping them in
the designated parking for a long period of time.

3 Temporary shelters/offices:

Porta cabins shall be removed properly including all utility connections therein.

4. Worksite:

All areas of the work site must be left clean and free of litters or waste materials.

5. Signboards, etc:

All safety and or directional sign boards that had been in used during construction stage
shall be removed and disposed off.

43.0 ABRASIVE BLASTING

This section covers blast cleaning using abrasives in the form of garnet, iron shot, grit, slag or
similar materials. Abrasive blast cleaning operators shall have pre-placement physical
examinations including X-ray and pulmonary function tests with repeat tests every year. The
abrasive blast cleaning operators shall wear an air supplied protective hood approved
(NIOSH/MSHA or equivalent) for the type of abrasive materials being used and any other
toxic contaminants (lead, zinc, etc) that are present. Where blast cleaning assistants could be
exposed to garnet, dust or toxic contaminants, then self-contained or air supplied breathing
apparatus shall be worn by the assistant. The breathing and air quality specification shall meet
the standards referenced in GI 1780.001-2 breathing apparatus. Personnel (eg. Maintenance,
workers handling abrasive supplies or clean up crews) within the range of rebound abrasive
contaminants shall wear approved (NOSH/MSHA or equivalent) dust filter respirators for the
specific airborne hazards, with periodic Carbon Monoxide (CO) alarm/detector.

A bonding system that bonds nozzle, hose, blasting equipment and the material being cleaned
shall be provided, and this bonding system shall be grounded to prevent a build up of static
charges. Ground continuity tests shall be conducted periodically to ensure proper grounding.

To prevent ignition of hydrocarbons in classified areas, the appropriate precautions listed


G12.100, Work Permit System shall be followed.

44.0 PAINTING AND COATING PROCEDURES

Workers engaged in paint and coating applications can be exposed to the dangers of fire,
explosion, chemical burns, toxic fumes, dust and insufficient air\. In short, no harmful
substances like paints and coatings shall be where an alternative is available. But if used, a risk
assessment will be carried out to determine the form of hazard and precautions to be taken.
Saudi Aramco General Instructions such as GI 2.100 (Work Permit System), GI 8.003
(Berating Apparatus) and Safety Requirements for Painting, SAES- H 102 will prevail.

1. All the workers shall be informed of the hazards involved and the precautions that
must be taken in handling paints.
2. The Project Manager shall ensure that engineers/supervisors understand the hazards,
precautions and emergency procedures, which will be documented in a method
statement.
3. The paint/coating container shall identify the chemical with symbols and works.
Hazards and precautions and material data sheet of the manufacturer will accompany
the container.
4. Toxic chemicals shall be stored in ventilated areas subject to restrictive entry.
5. Appropriate personal protective equipment or PPE shall be provided and worn by the
employees while handling paints and coating chemicals.
6. During the course of the work frequent checks will be made to verify precautions and
safeguards are in operation.

45.0 LOCK OUT AND TAG SYSTEM

The purpose of the lock out system is to render controllers, switches, valves etc on any system
(Electrical steam, hydro carbons, water, acid etc) inoperative and locked where the operations
of the control device could be hazardous to personnel working on the system.

Hold tags and Locks are primarily intended to protect the individual doing work from being
injured by an inadvertent start up.

Work Permit Issuers and supervisors shall ensure that the hold tags and lockout are used and
clearly noted in the work permit. The use of hold tags/lock outs system shall be strictly
enforced.

46.0 AIR POLLUTION CONTROL

All procedures shall be in accordance with SAUDI ARMCO Engineering Standard SAES A-
102 “Ambient Air Quality and Source Emission Standard”.

The following sources of air pollution emission shall be considered for the control in
construction phase.

• Dust (General)
• Combustible engine exhaust
• Abrasive blasting operation dust

Generally dust will be controlled by spraying water over areas where wind blow sand or dust
will affect personnel. The condition that necessitates spraying water will be judged and
determined by Construction Manager in consideration of weather, wind, ground dryness and
so on.

Exhaust gas from combustible engine shall be controlled by overhauling the engine
periodically and equipment which equipped with old/deteriorated engines shall be avoided to
prevent the emission of non- standard of exhaust gas.

Abrasive blasting operation shall be generally carried out within a facility that is covered by
sheeting to prevent spreading dust and blasting shot material out. By a certain reason if it is
difficult; to carry out the operation above stated condition and appropriate location that does
not affect and annoyance or inconvenience to nearby people or work shall be selected.

47.0 NOICE CONTROL

Noise emission from on site shall be regulated in accordance with SAUDI ARAMCO
Engineering Standard SAES-A 105.

Further to the above, contractor shall adopt a “Best Practicable Means” policy for the
minimization of the effects of noise during construction.

Should nose levels reach an unacceptable level, the following measures shall be taken.

• All employees of contractor/subcontractors entering noise hazardous are such as


construction site and workshop area shall compulsory wear ear plugs and use it as
required. Disposable earplug shall not be used more than one (1) day and the
construction supervisor is responsible to see that all used disposable earplug are
replaced daily.
• Minimizing of exposure time to high level of nose.
• Hearing protection equipment use is mandatory as required.
• Scheduling of inherently noisy operations to avoid disturbance to uses of facilities.
• Where necessary and practical, the use of an alternative equipment or plant.

48.0 SECURITY PROCEDURES

GCSCC employees will wear company uniform for proper identification. Long sleeves/
Safety Jacket with company logo at left side chest for rank & file employees as well as for
the staff employees.
Provision of color code safety helmet will be implemented, White for the Staff personnel
while blue &/ or yellow color safety helmet for the Rank & File employee to easily identifies
their category position.

No materials, tools or equipment will e taken out from the site without accompanying a
signed Material Gate Pass from the proper s required by GCS authorized Signatory on
Property Gate Pass procedure.

Where required a list of equipment, tools and materials brought in the work site, will be
submitted to client for record purposes.

49.0 DISCIPLINARY POLICY

It shall be the responsibility of the Site Manager to administer disciplinary measures for
safety rule violations.

General Requirements

1. Each employee who reports to the site for work shall be given a safety orientation.
The site safety program will be explained and safety responsibilities clearly
identified.

2. Disciplinary measures shall be administered in a fair and nondiscriminatory manner.


Disciplinary actions, including instruction for improvement to an individual, shall be
documented in that individual’s personnel file.

3. A violation of a project safety rule shall be promptly corrected. If necessary,


retraining shall be accomplished. A written warning shall be documented by the
employer with copies issued to (1) the individual, (2) the Site Safety officer (3) the
Site project Manager. This written warning applies to GCSCC and Sub-contractor
personnel.

4. A second safety violation by an individual will be cause for that individual to be


terminated from employment with eligibility for rehire.

5. A third safety violation committed by the same individual will result; in this
individual being terminated for cause and not eligible for rehire on the project.

6. Persons unintentionally violating safety rules, thereby being responsible by their


actions for accidents causing serious near misses, personal injury, death or damage to
property may be suspended from employment pending the outcome of a full
investigation of the incident.
7. . The results of the investigation will be a factor in determining whether the
individual involved will be terminated or cause, disciplined or returned to regular
work status.

8. Persons intentionally violating safety rules thereby being responsible by their actions
for accidents causing serious near misses personal injury, death, or damage to
property shall be terminated immediately.

9. Construction Supervisors are directly responsible for controlling all activities of craft
employees on construction projects. The play a key role in the execution and
maintenance of an effective jobsite safety program and they must plan their safety
activity with the same care and effort as they do with other portions of their work
program. Construction supervisors who incur any safety violations will receive a
written safety warning. However, depending on the gravity of the violation and its
consequence (s) this may cause his immediate termination and dismissal from the
project.
