As General identified gap at
 Medical Ward:
E There is staff skill and knowledge gap on how to teach the patients to enable
them to report their pain
E Poor documentation on vital sign as general
E All patients are unable to score their pain
E The new staff are not consider pain as 5th vital sign
E Senior still internalize pain mgt for patients is the pillar for patient satisfaction
E The way patients vital sign taken was not revised at round time
E Pain mgt issue was not raised on 3-9 meeting
 Emergency OPD
 No vital sign chart was inserted in patient card even if pt at EOPD
 Pain medication was given for pt with out scoring
 New staff were not informed on pain scoring and mgt
 Interval of vital sign taking also has series problem
 The way patients vital sign taken was not revised at round time
 Pain mgt issue was not raised on 3-9 meeting

 Even though they are senior 4/6 patient pain was not scored and managed ; two
of them are severe pain stayed more than 1 week

Ways Fore Ward
1. Train new staff within two weeks at service are for all wards
2. Conduct regular pain mgt audit
3. Cooperate with senior to revise pts vital sign at round time
4. Enable all staff to prescribe pain medication to improve patient satisfaction
