AFFIDAVIT OF UNDERTAKING

I, IRENE GRACE T. ACUIN, of legal age, Filipino, married, a resident of Alangalang,

Leyte, after having been duly sworn to in accordance with law, hereby depose and state: THAT -
-

1. I am directly hired by Presbyterian Support Otago based in New Zealand for the position
as care worker as evidenced by my employment contract.

2. I understand that under Sec. 125 of the 2016 POEA Rules Governing the Recruitment
and Placement of Landbased Overseas Workers (2016 POEA Landbased Rules), an original copy
of the verified/authenticated employment contract is one of the documentary requirements that I
must submit for the POEA to issue me an overseas employment contract (OEC);

3. Although I understand that the original copy of the verified/authenticated employment

contract is a requirement, I cannot submit the said document on the day of my appointment since
I just requested my employer to send me a copy of the original documents through a courier.

4. I request that I instead submit a scanned copy of my verified/authenticated employment

contract on the day of my appointment in lieu of the original document;

5. However, I undertake to submit to the POEA Direct Hire Assistance Division my original
verified/authenticated employment contract on or before December 30, 2022.

6. I hereby authorize to submit my original verified/authenticated employment contract to

the POEA Regional Center - Tacloban by the following individuals, namely:

7. I fully understand the contents of my affidavit, and that it was explained to me by POEA
personnel that submission of false or fabricated employment contract is a form of
misinterpretation which is a ground for administrative disciplinary action under Sec. 145 of the
2016 Landbased Rules.
