ROUTINE VENIPUNCTURE
(USING EVACUATED TUBES)
PROCEDURE 3 2 1 0
1. Secure requisition.
2. Knock on door before entry, greet, and identify self.
3. Identify correct patient.
4. Explain what you need to do and obtain consent.
5. Check pretest preparations.
6. Position and prepare the patient.
7. Perform hand hygiene and don gloves.
8. Assemble your equipment.
9. Double-check the expiration dates on the tubes.
10. Apply the tourniquet.
11. Select the site and palpate the vein.
12. Remove tourniquet.
13. Clean the site.
14. Attach the needle to the adapter.
15. Reapply the tourniquet.
16. Examine the needle.
17. Perform the venipuncture.
a. Anchor vein 1 to 2 inches below puncture site.
b. Insert needle (bevel up) at 15 to 30 degree angle.
c. Keep assembly angled downward.
18. Fill the first tube following order of draw.
19. Distract patient while filling tubes.
20. Advance and change the tubes.
a. Remove the tube when full.
b. If the tube contains additives, mix it by gently
inverting it 5 to 8 times.
23. Remove the tourniquet.
24. Remove the last tube.
25. Withdraw the needle.
26. Activate safety feature on the needle.
27. Apply a gauze to puncture site and apply pressure.
28. Dispose of the needle and other contaminated
materials in the biohazard container.
29. Label the tubes with permanent marker.
30. Attend to the patient.
a. Check site for bleeding, thank the patient.
b. Remove gloves and wash hands.
31. Deliver the specimen.

3. Identify correct patient.
4. Explain what you need to do and obtain consent.
5. Check pretest preparations.
6. Position and prepare the patient.
7. Perform hand hygiene and don gloves.
8. Assemble your equipment.
9. Apply the tourniquet.
10. Select the site and palpate the vein.
11. Remove tourniquet.
12. Clean the site.
13. Prepare the syringe.
a. Twist the needle onto the tip of the syringe.
b. Pull the plunger back then push it all the way.
14. Reapply the tourniquet.
15. Perform the venipuncture.
a. Anchor vein 1 to 2 inches below puncture site.
b. Insert needle (bevel up) at 15 to 30 degree angle.
16. Fill the syringe by slowly pulling the plunger.
17. Remove the tourniquet.
18. Withdraw the needle.
19. Activate safety feature on the needle.
20. Apply a gauze to puncture site and apply pressure.
21. Transfer the blood to the evacuated tubes.
22. Dispose of the syringe and other contaminated
materials in the biohazard container.
23. Label the tubes with permanent marker.
24. Attend to the patient.
a. Check site for bleeding, thank the patient.
b. Remove gloves and wash hands.
25. Deliver the specimen.

HAND COLLECTION USING A WINGED INFUSION SET

1. Secure requisition.
2. Knock on door before entry, greet, and identify self.
3. Identify correct patient.
4. Explain what you need to do and obtain consent.
5. Check pretest preparations.
6. Position and prepare the patient.
7. Perform hand hygiene and don gloves.
8. Assemble your equipment.
9. Remove the WIS from the package and attach to the
evacuated tube holder or syringe.
10. Insert the first tube and lay the assembly next to the
patient’s hand.
11. Position the patient’s hand and apply the tourniquet.
12. Select the largest and straightest vein and clean the
site.
13. Anchor the vein firmly with your non-dominant hand.
14. Grasp the needle between the thumb and the index
finger by folding the wings together in the middle.
15. Insert the needle into the vein (10-15 degrees), bevel
side up and lined up in the direction of the vein.
16. Gently thread the needle up the lumen of the vein
until the bevel is not visible, keeping the angle shallow.
17. Hold the needle in place by one wing with the thumb
of the opposite hand.
18. Collect the sample by pushing the collection tube into
the adapter.
19. Remove the tourniquet.
20. Remove the tube and invert as needed.
21. Withdraw the needle.
22. Activate safety feature on the needle.
23. Apply a gauze to puncture site and apply pressure.
24. Dispose of the WIS and other contaminated
materials in the biohazard container.
25. Label the tubes with permanent marker.
26. Attend to the patient.
a. Check site for bleeding, thank the patient.
b. Remove gloves and wash hands.
27. Deliver the specimen.
