AFFIDAVIT OF SUPPORT AND CONSENT

been duly sworn to in accordance with law, do hereby depose and state:

1. That I am giving full consent and authority to my mother, Emely Gannaban Leste,
to apply for passport with the Office of Consular Affairs of the Philippine
Department of Foreign Affairs on behalf of my child, Colin Leste Ocampo;

2. That I am willing and able to maintain and support my child financially and
guarantee that she will not become a public charge or burden during her stay in
the United States.
