AFFIDAVIT OF LEGITIMATION
I, ORLANDO D. LEGASPI and ANALYN C. LEGASPI, husband and wife
respectively, Filipino citizens, both of legal age, and residents of San Roque Street, District II,
Julita, Leyte, Philippines, after being duly sworn to in accordance with law, hereby depose and
state:
1. That we are the parents of “JOHN DAVE L. CALDA” who was born on JUNE 21,
1999 in DISTRICT II, JULITA, LEYTE; A copy of his birth certificate is hereto
attached as Annex “A” and shall form part hereof;

2. That our son “JOHN DAVE L. CALDA” was born out of wedlock;

3. That at the time our son was conceived and born, we both possessed no legal
impediment to marry each other;

4. That on August 9, 1999 we got married; A copy of our marriage contract is hereto
attached as Annex “B” and shall form an integral part hereof;

5. That by reason of our subsequent valid marriage, the above-named child shall ipso
facto be LEGITIMATED pursuant to the provisions of the New Family Code of the
Philippines, as amended by RA 9858;

6. That our son shall now be known as “JOHN DAVE C. LEGASPI”;

7. That we are executing this Affidavit to attest to the foregoing facts, for purposes of
complying with the requirements in applying for the legitimation of the birth status of
our son by subsequent marriage, for proper recording of such legitimation in his civil
registry document, and for all legal intents and purposes it may serve.
