Task: Survey questionnaire development

Design a 15-point knowledge, attitude, and Practice questionnaire on how general public in UAE dispose
unused medicines.
The questionnaire must have ‘Yes/No’ and ‘Likert type’ questions.
Follow standard procedures for questionnaire development such as content development, content
validation and reliability analysis.
The questionnaire must be made in google form and converted to QR code.
