AFFIDAVIT OF COHABITATION

We, Ryan jay L. Maata and Darlene C. Castillo, both of legal age, Filipino, Single, and
residents of Suburban, San Jose, Rodriguez, Rizal, after having been duly sworn to in
accordance with law, do hereby depose and state that:

1) We have been living together as husband and wife since 2011 for period of 11years;

2) We are both single statuses, without any legal impediment to marry each other;

3) We are executing this affidavit to attest to the foregoing facts and for the purposes of
contracting marriage without the need of securing the marriage license.
