GENERAL Deceased Name: DADISURIBABU Sex: Male Age: 47 Years Height: 165m Physique: Moderate Nutritional state: Moderate IDENTIFICATION MARKS: An identified dead body. The dead body was identified by PC No. 2214, K. Narsingrao of Anakapalli Traffic P.S. DEATH PARTICULARS AS PER THE INQUEST AND FORM 80 The death occurred at 10:52 PM on 03-07-2023 while undergoing treatment at KGH, Visakhapatnam. B, EXTERNAL EXAMINATION FINDINGS EXTERNAL APPEARANCE: The body is wrapped in a white hospital bed sheet & grey-white bed sheet. The body is supine, eyes and mouth are closed and arms are placed by the side of the torso. ECG lead stickers present on front of chest. There is nothing to suggest about other natural orifices. The dead body was preserved in cold storage prior to the autopsy. POST-MORTEM CHANGES: Temperature: Sub-normal. Post Mortem Lividity: Present on back of body. Rigor mortis: present in all four limbs. Putrefactive signs: Nil INJURIES: The following antemortem injuries are present on the body: 1. Surgical sutured wound of size 05cm x 0.Sem on right side of neck 2. Punctured wound of size 02cm x 01cm present on front of left chest OScm beside left nipple at the level of a" thoracic rib 3, Grazed abrasion of size 04cm x 02cm with scab formation present on front of left lower abdomen 4, Grazed abrasion of size O4cm x 02cm with scab formation present on left knee 5, Grazed abrasion of size 03cm x Oem with scab formation present O1cm below left knee 6. Grazed abrasion of size 02cm x 02cm with scab formation present Oicm below right knee 7. Grazed abrasion of size 01 x 01cm with scab formation present on dorsum of left hand 8. Multiple rib fractures of left chest at the level of T4-T8 thoracic ribs associated with contusion of surrounding soft tissues 9. Fracture of body of sternum present. 10. Laceration of size 06cm x O4cm present in the Inferior aspect of right lobe of liver 11. Rupture of spleen present associated with 1000cc of blood present P.Oc.
