BASIC JOB STEPS POTENTIAL HAZARDS RECOMMENDATIONS

1.7.2. White Helmets must be available on site

2. Mobilize tools and equipment to worksite 2.1. Back injury (manual handling) 2.1.1. Manual handling (Proper Lifting) shall be covered in
daily TBT and be implemented on site

2.2. Traffic and pedestrians 2.2.1. All tools and Equipment to display current colour
code
2.2.2. Audible reversing alarms shall be installed on
equipment/vehicles
2.2.3. Pedestrian safety precautions shall be discussed in
daily TBT
2.2.4. All traffic rules and signs should be followed (on site
speed limit is 25-km/h)
2.2.5. Only approved routes shall be used for access to and
egress from the site
2.3. Overturning of vehicle / Crash injuries 2.3.1. All drivers shall have valid driving licenses
2.3.2. The areas where heavy equipment are working shall
be clearly marked on site
2.3.3. Safe access shall be maintained at site for vehicles
and equipment
2.3.4. Use of seatbelts shall be mandatory at all times

2.4. Slips / Trips / Falls 2.4.1. Housekeeping shall be discussed in daily TBT and
be implemented on site on a daily basis

3. Manual Excavation 3.1. Heat Stress and Dehydration 3.1.1. Provision of sufficient cool drinking water (include the
use of ice)
3.1.2. Provision of shaded rest places (HEAT STRESS
SHELTER)
3.1.3. Frequent breaks during extremely hot weather
3.1.4. Tool box talks regarding heat stress

3.2. Excavated soil falling 3.2.1. Excavated material shall be placed at least 1.0m
away from the excavation
3.2.2. Covering by polyethylene sheet if necessary
4. Exposure / Protection of existing U/G services 4.1 Damage to COMPANY or existing property 4.1.1. Any defect, incident or near miss shall be reported to
the GAMA-HES Manager
4.1.2. No mechanical excavation shall be made nearer than
1 m of live plant equipment or existing underground
utilities
4.1.3. Location of U/G services shall be identified by a
cable detector operated by a competent person
4.1.4. U/G services shall be marked on site; and they shall
be barricaded with steel posts at each side
4.1.5. The safe work methods and hazards associated with
tasks shall be covered in daily TBT
5. Support and Protection installation for Existing U/G 5.1.Unauthorized work / Activity 5.1.1. The required Permits to Work shall be obtained and
cable Line kept at work area at all times along with the JSA
5.1.2. Review the request permit
5.1.3. Provide Permit Box

5.2. Electrocution 5.2.1. Obtain all necessary permit - Acquire Confined Space
Entry Permit (>1.2 meters depth)
5.2.2. All Cables must be treated live
5.2.3. Close supervision by a competent person
5.2.4. Warning tape and safety signs and precaution on site

5.3. Injuries due to collisions or unstable material 5.3.1. First Aid kits and certified First Aiders shall be
available at site clinic
5.3.2. All employees shall be made aware of Emergency
Procedures and must be discuss in TBT
5.3.3. Audible reversing alarms shall be installed on each
vehicle
5.3.4. Entry to the excavation area, which shall be properly
barricaded, shall be prevented when excavation is in
progress
5.3.5. Excavated material shall be stocked at least 1 meter
away from the edge of the excavation
5.3.6. Side slopes of excavations should be stable

5.4. Damage due to Unsupported services 5.4.1. Existing facilities / structures shall be supported as per the
instructions from competent person, if required
5.4.2. Sign boards and barricade (precaution) if necessary
5.5. Dust Control 5.5.1. Provision of dust masks for workers and ensure their
use
5.5.2. Provide dust mitigation by spreading water
5.5.3. The distance from the excavator bucket to the body of
the truck shall be reduced to an acceptable height (0.75
to 1 m)

Asphalt / Concrete cutting use of Cutting Machine 6.1 Injury (cut, loss of hearing due to noise), 6.1.1. Inspected and approved cutting machine (with guard,

8.1.2. Operators and Job Supervisor shall ensure that the
vehicle is not overloaded
8.1.3. All trucks shall be equipped with covering canvas with
ropes for easy (un)covering

8.2. Collisions / equipment falling into the excavation 8.2.1. No equipment shall work nearer than 1.5 times the
excavation depth from the excavation
8.2.2. Banks-men with high-visibility vest shall accompany the
activities

8.3. Speeding, Careless cause to damage existing 8.3.1. Operators shall be made aware of traffic rules and
facilities regulations; penalty system shall be implemented
8.3.2. Only approved transport route for truck and strict
implementation

8.4. Unauthorized equipment operator 8 .4.1. Only competent and certified operator shall operate the
equipment,
8 .4.2. When left unattended, the engine shall be switched off
and the keys removed from the ignition

8.5. Passengers on operating equipment 8.5.1. No passengers are allowed to ride on a single designed
cab (bobcat, loader)
