This application is available online. Please visit our public website at AzASRS.gov, log in to your secure myASRS
account to complete the online application.
The ASRS is pleased to offer you the convenience of electronic deposit of your monthly benefit.
Please note: The ASRS posts monthly pension benefit summary statements online at www.azasrs.gov. To access your
monthly statements, log into your ASRS secure member web account by clicking on the “Login Here” button.
Receiving your pay summary electronically is a convenient and secure way to receive financial information.
Follow these instructions to complete this Direct Deposit request form, however, the most secure way of providing the ASRS
with the information on this form is to log in to your secure member web account and submit this information online.
SECTION 1 – Direct Deposit Request - Check the appropriate box to indicate the type of authorization you are requesting.
Choose “Start a New Authorization” if you are a new retiree or have not previously set up a direct deposit account for your
monthly benefit.
Choose “Change Existing Authorization” if you would like to change the account number and/or banking institution of an
existing account. Do not close your old account until your first payment is deposited into your newly designated
account and/or banking institution.
Choose “Cancel Existing Authorization” if you would like to cancel your Direct Deposit Authorization of an existing account.
Your monthly benefit will be paid on an ASRS Prepaid Debit Card. Do not close your old account until you receive your
Prepaid Debit Card.

Disclosure of your Social Security number is mandated by Section 6109 of the Internal Revenue Code. The ASRS will use Social Security
numbers only to obtain information about an individual’s ASRS account and to inform the Internal Revenue Service about distributions and
withholdings with respect to the individual’s account.
