Use these forms to keep an open line of communication with your
students’ parents.

OOPS! – Copy on red paper. Send this note home to keep parents
advised of serious behavior issues with which you need parental support
to improve.

HELP PLEASE – Copy on orange paper. Send this note home when a
student is struggling with a concept and needs additional support at
home.

INCOMPLETE WORK – Copy on yellow paper. Send this note home


attached to incomplete assignments. This is perfect for use when
students are not attending to a task or when the task is too complicated
and they need additional help. It is good to keep parents advised of
work being too challenging so they are aware of their child’s needs so
they can help at home, too.
