A complete and true copy of the organizations constitution and by-
laws signed by all of its officers (for the new applicants) and
reviewed by the Committee on Student Organizations. For the
organization seeking renewal, the following shall be submitted:

● newly revised constitution and by-laws reviewed by the
Committee on Student Organizations
● list of new members and/or officers
Action/Strategic Plan or programs of activities for the whole year
5 with the corresponding date. Each organization must sponsor at
least one constructive activity/project every year.
Brief bio-data and three (3) 2x2 photographs of the president of the
6 organization.
A list of officers and members of the organization, indicating their
7 complete home and campus/lodging house addresses.
Organizational structure with complete names and pictures of
8 officers.
Certification signed by the registrar that all its members are
9 students of the university or compilation of individual registration
forms of student-members of the organization
A letter of acceptance/consent by at least one (1) faculty adviser
10 chosen by the organization.

Two (2) copies of the following reports for activities undertaken
during the immediate past year:
