JOINT AFFIDAVIT OF
TWO DISINTERESTED PERSONS

We, LEONOR T. TABINAS and RAMON S. CAMACHO, all of legal ages,

married, Filipinos, and residents of Brgy. Timbaan, and Rizal, San Andres,
Catanduanes after having been sworn to in accordance with law, do hereby
depose and state:

1. That We know personally the person of RAMEL BARDON, for being

our barrio mates and family acquaintance for a long time;

2. That the person of RAMEL BARDON as appearing in the in his

Baptismal Certificate issued by the Parish of St. Andrew the Apostle
and RAMIL BONA BARDON , as appearing in the Certificate of Live
Birth of his children Jessa Timanel Bardon with Registry No. 2012-733
and Meljane Timanel Bardon with Registry No. 2011-180 issued by
LCR San Andres, Catanduanes refers to one and the same person;

3. That We are executing this affidavit in order to attest the veracity of the
foregoing statements and to affirm that the same are true and correct
to the best of our knowledge and belief; and for the documentary
requirements of the Philhealth.
