KNOW ALL MEN BY THESE PRESENTS:

This Deed of Extra-Judicial Settlement is made and executed this __ day of

W I T N E S S E T H;

WHEREAS, the HEIRS are the surviving wife and the surviving legitimate

_____________; copy of his Death Certificate is attached as Annex “A”;

WHEREAS, the deceased at the time of his death was a resident of

____________ and left the following properties:

WHEREAS to the HEIRS’ knowledge, the deceased left no will or testament, nor
any debt or unpaid obligation whatsoever to whomsoever at the time of his death.

WHEREAS, ONE-HALF PORTION of the above described properties belong to

the Surviving Spouse as her conjugal share thereon; thus, only the OTHER ONE-HALF
PORTION thereof forms part of the estate of the said deceased.

NOW THEREFORE, pursuant to Section 1 of Rule 74 of the Revised Rules of

Court of the Philippines, the HEIRS having legal capacity to contract, by these presents,
being the only surviving HEIRS of the deceased ____________, do hereby settle and
adjudicate unto themselves the ONE-HALF PORTION of the above described properties
in TWO EQUAL SHARES, at ½ pro indiviso share each.
