TL1451A Protection works when the device powers off

My customer found protection works unexpectedly when the device power is turning on and off repeatedly for approximately
1s. While in protection mode, it is impossible to control at all, but return to normal work after power cycling.

After changing DTC capacitor value from 1uF to 0.1uF, the phenomenon has been disappeared. But, it may may not be a
corrective action.

(1) What's the cause to enter the protection mode?

(2) Is changing DTC capacitor value to 0.1uF a corrective action on this?

(3) Can you please let me know how to calculate the Dead-Time?

That part is over 30 years old. I doubt there is much supplemental information available for it. Also, it is not clear from
your partial schematic how the part is being used. Can you provide more details?
