I/We, hereby request you to convert my/our existing Resident Savings Account (where I am the sole/ first account holder
held under Customer ID No………......................………………. to Non-Resident Savings Account - NRO Account.

I/We hereby agree, confirm and declare the following:-

1. That the above account(s) shall be re-designated as NRO Account(s) on the basis of the declaration made by me/us subject to submission of the
required documents. However, the account number of the re-designated NRO Account(s) will remain the same as my existing Resident Savings
Account (s) Number
2. All active Fixed Deposits (except Tax Saver Fixed Deposit) which are linked to my resident savings account shall be closed and re-booked for the
remaining tenure and at the original contracted rate of interest
3. All Recurring Deposits which are active and linked to my resident savings account shall be closed and credited to my re-designated NRO Savings
Account
4. That I/we will not be able to avail of Double Taxation Avoidance Agreement (DTAA) in the current financial year. The same can be availed by
me/us in the next financial year by submitting the necessary DTAA documents as per the Bank's requirement.
5. I hereby confirm that all unused cheques have been destroyed by me/us. I/we also authorise the bank to destroy all unutilized cheques, if any.
6. That all the existing Domestic/International Debit Card in my / our name shall be de-activated and if requested by me/ us new Domestic Debit
Card for my/our re-designated NRO Account shall be issued by the Bank.
7. That I/We hereby undertake that the change from Resident to Non-Resident status will be informed by me/us to NSDL for updating my/our KYC
details for Mutual fund transactions/holdings in my/our name, if any.
8. That I/We will close Investment Services Account and open NRI Investment Services Account
9. That I/We will submit all the documents, forms and details as required by the Bank for converting my /our existing Resident Demat Account to
NRO Demat Account
10. That, for all the accounts wherein I am the primary account holder in an account held jointly with a resident then my status will remain NRI, but the
mode of operations will be changed to Former or Survivor.
11. That, for all the accounts wherein I am a joint account holder and the Primary Account holder is a Resident, then I shall submit the name deletion
request as per the requirement of the Bank.
12. That, the TDS will be applicable as per my/our new residential status from the last interest compounding date. TDS for current quarter shall be as
per existing status of the customer. (Status for TDS deduction will as on date of accrual or TDS deduction whichever is earlier). As an Assesse
I/We shall be responsible to file returns as per my/our tax status
13. That, I shall abide by all the terms and conditions as applicable on the NRO Account(s) and as amended from time to time by YES Bank at its sole
discretion.
