I work safely in confined spaces

I have a valid Permit to Work for confined space before


entering.
I follow the Permit to Work requirements.
I confirm that all relevant energy sources are isolated.
I confirm that the atmosphere (air space) has been
tested and is continually monitored.
I inspect and use my breathing apparatus when
required.
I ensure an attendant is standing by.
I check the rescue plan is effective and in place before
entering.

I FOLLOW WORKPLACE & DRIVING RULES


I follow workplace & driving rules

I never work or drive under the influence of drugs and /


or alcohol.
I always wear my seat belt in vehicles.
I observe speed limits and reduce my speed based on
road, weather conditions and road signage.
I am fit, rested and fully alert while working & driving
and stop when I am tired.
I check and follow any travel management plans.
I do not use a cell phone while driving except in cases
of emergency.
I obtain authorisation before overriding or disabling
safety-controls, deviating from procedures or crossing
a barrier.

I ISOLATE ALL TYPES OF ENERGY


I isolate all types of energy

I have identified all types of energy sources.


I have ensured that the hazardous energy sources have
been isolated, locked and tagged.
I have tested to verify the lock out is effective.
I have checked that there is zero residual or stored
energy.

I WORK SAFELY AT HEIGHT


I work safely at height

I have a valid Permit to Work / authorization when


working at height outside a protected area.
I wear suitable fall protection and inspect the
equipment before use.
I tie off 100% to approved anchor points.
I secure tools and work materials to prevent objects
falling.

I WEAR PERSONAL PROTECTIVE EQUIPMENT


I wear personal protective equipment

I use all the personal protective equipment necessary


for my tasks as described in the risk assessments.
I wear a personal flotation device (PFD) in areas with
there is a risk of falling into water.
I wear a helmet and hi-visibility reflective clothing
while riding a motorcycle.
I wear hi-visibility jacket when working around mobile
equipment.
I wear a harness with a life-line attached when
entering containment areas where the material is
uneven, cone shaped or funneled.
I wear respiratory protection when there is a risk of
atmospheric contaminant or oxygen deficient
atmospheres.

I WORK WITH A VALID PERMIT WHEN REQUIRED


I work with a valid permit when
required

I have confirmed if a Permit to Work / authorization is


required.
I am authorised to perform the work.
I understand the content of the Permit to Work /
authorization.
I have confirmed that hazards are controlled, and it is
safe to start.
I stop work and reassess if any risks or conditions
change.

I STAY OUT OF THE LINE OF FIRE


I stay out of the line of fire

I avoid standing or position myself in the way of moving


or falling objects.
I ensure I am always visible or out of the way of moving
vehicles.
I stay out of the path of stored energy.
I stay out of the way of items or objects under tension.
I never place hands or any body part in equipment that
can rotate, move of turn.

I CONTROL FLAMMABLES AND IGNITION

I control flammables and ignition
sources

I identify and control all sources of ignition.


I always use intrinsically safe devices in workplaces
with explosive atmospheres.
Before starting any hot works:
I remove all flammable materials from the work area.
I obtain Hot Works permit before starting.
Before starting any hot works in area of potential
flammable atmosphere:
I have isolated any gas sources.
I have ventilated the area & tested the air.
I will continuously monitor the air.

I CONTROL THE RISK OF ENGULFMENT &


SUFFOCATION
I control the risk of engulfment &
suffocation

I identify any potential engulfment risk areas.


I confirm that safe walking areas are clearly marked.
I do not work in areas where piles of materials could
collapse or shift (I will stay out of the Line of Fire).
I do not enter any ground excavation unless it is
properly shored / supported.
I confirm that an attendant is standing by for emergency
extraction.
