Introduction
Care plans are fashioned to provide detailed layouts of how patients will projective and
proactively handle them through identifying medical issues affecting them and the remedies
formulated to better their health. This paper intends to vividly discuss a care plan for two
diseases predisposed on patients. These diseases include hypertension and obesity, presented in
the care plan as areas of concern. The epicenter of the discussion focuses on preventing the
conditions and promoting better health in patients. Areas covered in this care plan include
evidence-based intervention, referrals for patients diagnosed and affected by type two diabetes
Health Problems
The identified health concerns include obesity and high blood pressure. Ogaden et al.
(2020) point out that obesity is perceived as one of the complex diseases seen through the
excessive fat content within the body. Obesity increases the chance of the body becoming
predisposed to other secondary illnesses like cancer, high blood pressure, and diabetes. Fat
accumulation within the body blocks and hinders body fluids' proper movement, including blood
within the blood vessels. The fat also chokes other body organs limiting their proper functioning
and creating vast struggles within the body o operate efficiently. Type 2 diabetes, also seen as a
prevailing condition in the care plan is caused by the patient's overweight due to the excessive
accumulation of fats and energy within the body. The patient's body struggles to regulate
excessive amounts of sugar, thus escalating the condition further escalated b the overweight.

The goal of the care plan is to vividly and extensively promote health in the patient and

Through the exposure of the patient to proper diet and physical therapy, it is projected
