Health Promotion Care Plan

Introduction

Care plans are fashioned to provide detailed layouts of how patients will projective and

proactively handle them through identifying medical issues affecting them and the remedies

formulated to better their health. This paper intends to vividly discuss a care plan for two

diseases predisposed on patients. These diseases include hypertension and obesity, presented in

the care plan as areas of concern. The epicenter of the discussion focuses on preventing the

conditions and promoting better health in patients. Areas covered in this care plan include

evidence-based intervention, referrals for patients diagnosed and affected by type two diabetes

and obesity, and goals for the care plan.

Health Problems

The identified health concerns include obesity and high blood pressure. Ogaden et al.

(2020) point out that obesity is perceived as one of the complex diseases seen through the

excessive fat content within the body. Obesity increases the chance of the body becoming

predisposed to other secondary illnesses like cancer, high blood pressure, and diabetes. Fat

accumulation within the body blocks and hinders body fluids' proper movement, including blood

within the blood vessels. The fat also chokes other body organs limiting their proper functioning

and creating vast struggles within the body o operate efficiently. Type 2 diabetes, also seen as a

prevailing condition in the care plan is caused by the patient's overweight due to the excessive

accumulation of fats and energy within the body. The patient's body struggles to regulate

excessive amounts of sugar, thus escalating the condition further escalated b the overweight.

Whelton et al. (2018) explain that high blood pressure, the second disease, is a risk factor for

stroke and heart disease. The patient is thus at risk of the stated disease due to hypertension and

obesity within their body system.

Goals

The goal of the care plan is to vividly and extensively promote health in the patient and

prevent disease or escalation of prevailing conditions within the body system. The care plan

discussed interventions like lowering the patient's body weight through exercise and lowering the

body's blood pressure to combat obesity and hypertension. By the end of the treatment, the

patient should have reduced his or her body weight by 10% solving the obesity issue while

having a lowered blood pressure that is less than 130/80mmHg.

Evidence-based Intervention

Obesity can be solved by looking at and recommending a probable diet and involving the

patient in physical exercise. The proposed diet should have less calorie content to mitigate fat

intake and build-up in the body. Swift et al. (2018) emphasize that the patient should also be

exposed to physical activities for not less than 150 minutes every week to help reduce his or her

body weight.

Hypertension is mitigated through medication and modification of lifestyle. The patient

should be exposed to hypertension medication to help lower blood pressure. The patient should

also check on lifestyle choices and avert sodium-laced food consumption while increasing

physical exercise to mitigate high blood pressure.

Referrals

To mitigate the two identified conditions, the patient should be referred to a professional

and approved dietitian and a physical therapist. The dietitian's role is to recommend and

formulate essential and realistic diet plans for the patient to curb obesity. The physical therapist

will advise on the physical programs the patient should undertake to become fit again.

Evaluation/Reflection

Through the exposure of the patient to proper diet and physical therapy, it is projected

that there will be a positive record of health status from the patient. Additionally, by following

all the recommended medication and ensuring they keep a healthy lifestyle, the patient will

recover and be healthy.
