CERTIFICATE ON DISABILITY

This is to certify that, Marasigan, Medelyn C., born on February 14, 1995, age 28, resident of 638M.T.
Halili Street, Sampaloc Manila, Metro Manila, in the region of NCR had voluntarily submitted herself to
this facility/clinic/office with regard to the nature of disability due to functional limitation currently
experienced by the herein patient.

Based on the personal interview and medical assessment conducted by herein physician, the patient is
diagnosed with Juvenile Idiopathic Scoliosis; accompanied by difficulty in standing and walking for
prolonged periods; writing and typing for prolonged periods; carrying heavy loads and therefore
considered as person with Orthopedic -Musculoskeletal Disability as classified by the Department of
Health Administrative Order No. 2009-011.

This Certification is issued on July 17, 2023 at De Los Santos Medical Center in compliance with the
requirement in the issuance of ID for the twenty percent (20%) discount for Persons with Disabilities
mandated by Republic Act No. 9442 or Magna Carta for Persons with Disabilities.
