Police Department, representing the proposed negotiated settlement of this action.

along with a District Attorney’s release or a Certified Criminal Court Disposition

from the District Attorney’s Office or Court Clerk in the borough of arrest. If you
have not received any correspondence within ten business of submitting the forms,
