All students of the College of Nursing are required to attend a set number of games, which will be reflected in the index card each student will prepare. b, The students may attend any face-to-face Nursing game so long as they are included in the Intramurals 2023 schedule, and have faculty instructors supervising them. Section 2. The Index Card "COLLEGE OF NURSING. UNIVERSITY WEEK AY. 2023-2024 NAME: YEAR & SEC: PARTICIPATION: PLEASE INDICATE HERE IF VOU ARE IN A SPORTS TEAM AND POSITION (EX. VOLLEYBALL PLAYER / COMMITTEE) oF N/A EVENTGAME NAME OF C1. SIGNATURE a. All students of the CDU College of Nursing are to prepare a 1/2 index card. b._ The index card must be formatted following the picture above. d. The index card must be brought to every game/activity throughout the entire Intramurals week. fe. By the end of the Intramurals, the entire index card must be filled out with signatures (see section 3 for specific guidelines). Section 3. Collection of Si atures & letion of Requirement ‘The students may gain each signature from the CDU-CN Clinical Instructor supervising the game. The students may identify the CDU-CN clinical instructor if they're wearing 2 red nursing shirt/polo, and an ID (Faculty-in-charge). |. Awit Sayaw Is NOT included in the Index card signing because this event has limited capacity. J. Asstudent is only allowed to collect 2 signatures in the moming, and 2 signatures in the afternoon of one day. A maximum of 4 signatures per day is allowed. k. In order to be eligible to obtain the signature of the Clinical Instructor, the student must watch the game from the beginning until the end. |. After the Intramurals has ended, the student must present the completed Index card to the CDU-CN S80 for final signing by the SBO Treasurer (to signify that the student has paid the council fee for the semester), and the SBO President (to signify that the requirement has been completely met). m. Requirements to obtain the signature of the SBO Treasurer and the SBO president are the following: i. Assisting the faculty-in-charge of the game. 2. Games handled by this committee are NOT limited to only the College of Nursing, but also includes all other college games (such as Dentistry vs. Medicine and other interdepartmental games). ii, Participation for this committee: 1, Games & Sports Comm, - Student Volunteer v.
