    Declaration form filled by individuals applying for surrender of Indian passport/Citizenship, when
    original Indian passport is not endorsed or the certificate of submission from the foreign authority
    containing the India passport number with the application.

    German Authority/Any other foreign Authority_______________________ on ________________, while
    acquiring German/ Other nationality. I am unable to furnish a certificate of submission of my
    Passport to the German Authority/Any other foreign Authority ____________________, as the Authority
    has expressed its inability to issue such a certificate.
