AFFIDAVIT OF UNDERTAKING

1. That, I am in support of the project of the Camarines Norte Police Provincial

Office dubbed as “TANGLAW LAHI” with a mission to provide scholarship program to
deserving young students of Camarines Norte specially those students that are stricken
by poverty to provide for them better future;

2. That, necessary fundings and monetary support is needed to realize this

mission thereby, I herewith voluntarily give an amount of Php _______________as
support with the noble project of the CNPPO.
