UNIFIED SCHOOL REPORT CARD (SF9) FOR GRADES 1-6 (ELEMENTARY}


AND GRADES 7-10 duNIOR HIGH SCHOOL (JIIS)

1. In line with the issuance of School Report Card, all Public Schools District
Supervisors, elementary and secondary school heads are directed to use the hereto
format/template/sample for a Unified School Report Card |SF9) for Grades 1-6
(ELementary} and Grades 7-10 .unior High School (JHS).
2. The format/template /sample of the sF 9 is in adherence to the downloadable school
forms of DepEd website. Learning areas are aITanged based on the Learners Information
System (LIS) with the inclusion of the Mother Tongue in the elementary.

3. DepEd Advisory dated November 18, 2021 (signed by Diosdado M. San Antonio -
Undersecretary for Curriculum and Instruction) on the inclusion of Homeroom Guidance
in the SF-9 (Report Card) and SF-10 (Forml37) as stated in DM-OUCI-346 or the Revised
Implementation of Homeroom Guidance (HG) during Crisis Situation for SY 2021-2022,
advised I.hat Homeroom Guidance letter grade shall no longer be reflected in the SF9 -
(Kinder/JHS/SHS) but the Homeroom Guidance Learner's Development Assessment
Form shall be in attachment to the SF-9 (Report Card) and SF-10 (Forml37).

4. For uniformity of said documents, please observe the fouowing;

• SF 9 - shall be in short or letter size - (front and back) - preferably use


vellum paper, with Deped and Division logo - (no school logo)
• Homeroom Guidance Learners Development Assessment Form shall be
printed in short or letter size -in a thick bond paper (front and back)

5. Relative to this, Pilot Schools of MATATAG Curriculum are advised to wait
for further instructions from DepEd Central Ofrice.
6. Immediate and wide dissemination of and compliance with this Memorandum is
directed.
