    Dear Dr. Maria B. Villaluz,

    In accordance with the conduct of gathering our primary data in your facility, Cebu Maternity
    Hospital, the researchers would like to gather the data six days a week until we can accumulate
    the target sample size. Each day starting from August 16, 2023 up until September 30, 2023, we
    would contact the facility or come by to see if there would be respondents that meet the criteria
    for our research study. Below would be the schedule and names that would conduct the research
    interview on each specific day.
