My name is Siddharth Shukla, currently working as a Business Analyst with an Insurtech Startup
Companjon Services DAC which has its headquarters in Dublin, Ireland. I currently hold a Stamp 1
Irish Residency Permit.

I require a short term multi entry tourist travel visa (Schengen) for my planned trip to Germany and
Italy between 16th August 2022 – 21st August 2022. I would request for the Visa duration ideally to
be valid for 6 months so that I can plan future travels into the Schengen area again.

 Confirmation of the Service Fee Payment.
 Travel Arrangements: Flight Tickets for my trip to Germany and Italy along with the
confirmed Accommodation bookings.
 Letter of approval from my current Employer.
 Recent Bank Statements.
 Latest Salary Payslips.
 Copy of Health Insurance.
 Copy of Travel Insurance.
 Copy of Valid Passport.
 Copy of Previous Shengen Visa.
 Copy of Irish Residency permit.

I hope that the above-mentioned documents are sufficient to grant me the requested entry to the
Schengen area.
