AFFIDAVIT OF DISCREPANCY

I, GEALDINA NADONGA AGUSTIN, of legal age, Filipino, married and a

resident of 30 Edison Tech University Hills Subdivision, Caloocan City,
Philippines, having been duly sworn in accordance with law, hereby depose
and state:

That my middle name in the passport was misspelled "NADONGGA"

instead of "NADONGA" as the true and correct spelling in the Certificate
of Live Birth as recorded in the Office of the Civil Registrar of Malabon
Rizal, Philippines;
That my true middle name is "NADONGA"; as appearing in the same
Certificate of Live Birth as my Maiden Name;
That I am executing this Affidavit of Discrepancy to attest to the truth of
the foregoing facts and to explain the discrepancy in my passport as
regards to my correct middle name.
