What is the purpose of this form?

To assist the EcoFads team in selecting the right candidates for the Internship Program.
This form may be used to identify candidates to be interviewed. Since all the applicants can’t be
interviewed, you need to fill in this form completely and accurately.

Minimum Hours:
Note: You need to devote 5 hours minimum per day for the Virtual Internship (Flexible- 2-3 hours in
between 10 AM-6 PM and 1-2 hours in between 7 PM-12 AM); 6-7 hours per day for a full-time
internship.
