Job Plan - Participants with Mutual Obligation Requirements


This Job Plan includes attending appointments and other activities that I agree to undertake that will
assist me in finding a job. I am aware that if I can't attend my appointments, or activities, I must
contact my Transition to Work Provider (or Services Australia) before the appointment or activity is
scheduled to occur.

I am aware that I have mutual obligation requirements that I must undertake in return for receiving
my income support payment, including the need to participate appropriately in Transition to Work. I
also understand that if I don’t participate appropriately I may be exited from Transition to Work by my
provider. Following that exit, my income support will be suspended by my Workforce Australia
Services provider.

I understand that I can have my Job Plan reviewed at any time to reflect any changes in my
circumstances and ways my provider will help me.

Job Plan Activities

I agree to attend appointments with my provider to discuss my job seeking progress, as required. My
provider will notify me when those appointments will occur.
1 hr weekly

I agree to search for work by contacting 10 employers per month, including any to which my provider
refers me. I agree to report and provide evidence of these job search contacts to my provider as
directed by the 16th of each month. I am aware that I need to look for any suitable work, not just
work that I would like to do.
2 hrs weekly job searching

I agree to undertake the neccessary activity in order to obtain my Provission licence to increase my
employability.
7 hrs weekly driving lessons

I agree to research potential employers and prepare job applications with the help of YES from
08/03/2023 to 30/06/2023.
2 hrs FN jobs club

Provider Assistance

Provider Services Assistance - MA will provide assistance with job applications, selection criteria
and resume preparation; post placement support, reverse marketing, & mentoring.

Training - Courses Assistance - MA may assist with the cost training courses and or tickets to
assist with employment opportunities.

Work Related Clothing and Presentation Assistance - MA TtW may assist with clothing/PPE for
employment or job interviews and other hygenie necessities

Transport and Licensing Assistance - MA TtW may assist with transport & licensing & up to 3x
driving lessons. MA TtW may assist with transport costs to help get to job interviews or work

I understand that participation in Transition to Work fully meets my mutual obligation requirements. I
also understand that if I don't comply with the requirements of this Job Plan that I may be exited from
Transition to Work.

Information You Need to Know
Under social security law, people with mutual obligation requirements are generally required to enter into a Job
Plan which is appropriate for their circumstances and participate in activities that are designed to assist them
into employment.

What should I do if I can't do the things I have agreed to?

If you can't do an activity listed in your Job Plan or can't attend an appointment that has been arranged for you,
contact your Transition to Work Provider as soon as possible. If you do so your provider may make another time
to attend your activity or appointment. If you repeatedly fail to attend appointments or participate in agreed
activities without an appropriate reason, your provider may exit you from Transition to Work.

What happens if I get some paid work?

If you or your partner has undertaken any paid work then you need to tell Services Australia about any income
you or your partner have received, in the same fortnight you worked. This will help Services Australia to make
sure you are paid the correct amount of income support. If you are overpaid, you may need to pay the money
back to Services Australia.

What happens if I leave a suitable job or I am dismissed from a job due to


misconduct?

If you leave paid work without a reasonable excuse, or are dismissed because of misconduct, you may not be
eligible to receive income support for up to eight weeks (or 12 weeks if you are receiving relocation assistance)
if you claim for, or are receiving, an income support payment with mutual obligation requirements.

What if I disagree with a decision that has been made?

If you disagree with a decision that Services Australia has made which impacts on your payment or a decision
Services Australia has made about your Job Plan, or have concerns about the service you have received, you
should contact the Services Australia feedback and complaints line as soon as possible on 1800 132 468.

If you disagree with a decision your provider has made about your Job Plan, or have concerns about the service
you have received, you can contact your provider to discuss your concerns or you can contact the Department of
Employment and Workplace Relations National Customer Service Line on 1800 805 260, who will investigate
your concerns. If you are not satisfied with the response you receive, you may take the matter further by
contacting the Ombudsman's Office on 1300 362 072.
