able to give parental / legal consent for my child to attend the concert on __________ 2023.

I confirm that I have read all the shuttle guidelines, information, schedules, terminal locations, terms, and
regular updates provided by Two Teen Tours on their public Telegram channel and undertake to abide by the
obligations which it imposes on me as the parent / legal guardian of the above-named child.

I acknowledge that Two Teen Tours is not responsible for providing adult supervision for my child or obligated
to regularly update guardians of the status of travel.
