My imaginary Non profit organization’s name is Clinica La Paloma.
Our mission is to provide healthcare equality to a rural community in CA of underserved
people ranging from children to seniors. Our goal is to provide and restore good health to the
community’s underprivileged population. Our community healthcare organization provides a
bicultural staff that provides the best treatment to people of the community who are underserved
due to a lack of resources and that struggle with cultural and language barriers Our goal is to
educate the community on the benefits of a healthy lifestyle as well as providing the highest
quality of medical care. We are a Federally Qualified Health Center. Our non profit
organization is composed of 400 staff members. We currently have 12 members in our board of
directors. We currently are serving 7000 patients a day.
This grant request will be to provide easier access to our underserved community by
enhancing their digital telehealth literacy. With the start of the pandemic of COVID 19, much of
the community struggled to get healthcare they needed. This brought the issues of telehealth
disparity within our community. Now that many healthcare offices are offering telehealth
services, it is a new era in which telehealth services will continue to increase in the future and it
is our duty to help our community not fall behind. Many members of the community do not have
the means for connection and/or the technological literacy for telehealth consultations. Much of
the community faces the struggle of coming into their follow up appointments due to
transportation issues. Our health center's goal is to use the resources needed to help minimize
and/or eliminate telehealth inequities. For example, we want to educate the community in
increasing their use of video telemedicine, providing assistance to patients who face digital
difficulties, and offering telehealth access for patients whose English is not their first language.
Our total budget cost for this health-care community project will be $60,000.
Staff Costs : $38,000, Travel: $3500, Equipment $6000, Training $4500 Consultant: 3,500,
Other Costs: $4500.
The grant our team is applying for is the The Connected Care Accelerator: Equity
Collaborative program. They provide grants to organizations in California to rapidly design, test,
scale, and improve access to telehealth.The goal is to reduce telehealth disparities within
underserved communities. They are offering a $75,000 grant. For the chosen participants, they
will receive access to monthly coaching, an online learning community, virtual convenings, and
the tools necessary to reduce and/or eliminate unequal access to telehealth in underserved areas.

The communities who would benefit from this grant would be the underserved
populations. In my fictitious non profit we serve more than 60% Medi-Cal and uninsured
patients. In order to apply the steps taken would be to first attend a webinar meeting and ask the
appropriate questions, secondly, submit an application online, thirdly submit the application
narrative questions which is composed of 8 questions asking in regards to the goals, data
reporting, and management of the project, fourth, submit the budget proposal explaining the

costs of the project. I will need to attach a recent w-9 form and a Leadership Support Letter that
comes from the CEO, CFO, or CMO.
