Team Peer Evaluation Form

Students will complete a confidential peer evaluation providing feedback on how each team
member performed throughout the project. These scores will be applied to the overall team score
to determine a final individual score. For the sake of clarity, if a team of 3 students achieved 80%
on the project and two students were given peer evaluation scores of 100% and one student was
given a peer evaluation score of 50% the final individual grades would be 80% for students 1 and
2 and 40% (80% x 50%) for student number 3. The final individual grade will be determined by
the instructor and is to be kept confidential.

Carefully evaluate the performance of each member of your group, including yourself, over the
period of the group project. Use 0 to 100% for each category.
