 NUTRITION ASSESSMENT
Past MedicalHistory:
Patient had no substanstial past medical history
Physical Examination:
The patient has significantly loss weight due to her condition and is also manifesting
other hyperthyroid symptoms like palpitations, heat sensitivity, and anxiety.
Laboratory Data:
Thyroid Function Tests: Elevated free T4 and T3, suppressed TSH.
Thyroid Ultrasound: Showed a diffusely enlarged thyroid gland with increased
vascularity, consistent with Graves' disease.
Thyroid Antibodies: Positive for thyroid-stimulating immunoglobulin (TSI) and anti-
thyroid peroxidase antibodies (TPOAb).

 FOOD INTAKE:
The patient used to drink coffee at least twice a day and dairy foods such as
chocolate and yogurt prior to her current condition.The client’s appetite has amplified
noticeably after her confinement, which is a common occurrence reported among people
diagnosed with Graves’ Disease who often report constant weight loss regardless of their
increased appetite as the body continues to break down fuel at a rate faster than it can
replenish it (Rowcroft, 2022)..The patient’s physician advised her to eat at least three
times a day, following her present dietary prescription of avoiding foods high in iodine

 NUTRITION DIAGNOSIS
PES Statement:
Imbalanced Nutrients related to metabolic disorders as evidence by patient
diagnosis of Grave’s Disease

 NUTRITION INTERVENTION
Patient EGJ was advised eat a low-calories diet that is still high with essential
nutrients which are high in antioxidants to keep her immune system strong and calcium
rich foods as Grave’s Disease may weaken the bones; Low-fat protein, while she has to
avoid refined, sugary foods, caffeine, foods with high iodine levels.The client’s
physician advised her to eat at least three times a day, following her present dietary
prescription of avoiding foods high in iodine.

 NUTRITION MONITORING AND EVALUATION


After the procedure, her dietary intake will be observed utilizing a 24-hour food
recall. It is critical to regularly monitor her calorie, protein, sugar, vitamin, and mineral
intake. These figures will allow us to evaluate the success of nutrition education and
identify whether she is eating a balanced diet.
