RE: Order Form with Authorization to Debit for Secondary Market

This is to confirm my/our request for BPI Capital Corporation (hereinafter referred to as “Broker”) to Purchase Sell
the following securities (the “Securities”) on my/our behalf:

It is understood that the execution/implementation of this order is on a “per allocation” basis, and there is a required minimum size and
denomination for the purchase of securities.

I/We acknowledge that this order is being executed and transmitted after the Selling Agent or its representative has duly explained to
me/us the features of the Securities subject of this order including the benefits and risks involved that I am/we may be exposed to. In this
regard, I/we also confirm that I am not prohibited to hold the Securities under any offering circular or similar document in relation to the
Securities or a person whose acquisition, holding or transfer of the Securities would violate any applicable law or regulation.

This order is given on a non-discretionary basis and I/we acknowledge that I/we have exercised my/our own judgment based on my/our
sufficient understanding of the transaction and the risks attached thereto.

Upon execution/implementation of this order, I/we commit to promptly and timely deliver any documents, letters of instruction, payments,
and such other requirements that may be needed to complete the settlement/delivery of the Bonds.

This order, serves as my/our instruction and authorization for the Selling Agent to undertake or cause to undertake the debit before the
settlement date our BPI SA/CA # ______________ , for the total cost related to the purchase/sell of the Securities, including broker’s fee and/or

I acknowledge that (a) the broker’s fee and admin fee (for coupon bearing bonds) applicable to my purchase of the Securities
have been properly explained to me and I understand that I am liable to pay the relevant amount in the manner calculated above
and that (b) if my/our nominated Settlement Account is my/our Current/Checking Account, I/we shall ensure that the account is sufficiently
funded to cover any and all checks drawn and/or payment instructions issued against the Account including this purchase order. For this
purpose, I/we agree not to hold the Selling Agent and/or the Bank of the Philippine Islands liable for the return of any insufficiently funded
check(s) or for any insufficiently funded purchase order.

In addition to the provisions of Section 14 of the Client Agreement on Limitation of Liability, Indemnity and Further Assurance, I/we
agree to hold free and harmless BPI Capital Corporation, its directors, employees, service providers, representatives and agents from any
and all claims, liabilities, damages, suits or causes of action of whatever nature or kind (including the cost of defending or settling any
action, claim or demand), in the event of a failed trade, unless such failure is caused solely by BPI Capital Corporation’s evident bad faith
or gross negligence. Without prejudice to my/our rights under Section 20 of the Client Agreement on the Cooling Off Period, I/we further
agree to indemnify BPI Capital Corporation for any and all costs, charges, taxes, fees, or losses associated with my/our failure to settle a
trade on the agreed Settlement Date or such other losses or damages resulting from this Order Form.

This order is good until____________________unless sooner cancelled in writing.

I/We acknowledge that the Selling Agent is authorized but not obligated to automatically reinvest in my/our name the proceeds of any
maturing placement, under such terms or conditions that the Selling Agent deems appropriate, unless I/we provide to the Selling Agent
before such maturity a duly acknowledged written investment instruction which specifies acceptable placement terms. If my/our Securities
mature without any written investment instruction having been received and acknowledged by the Selling Agent, I/we acknowledge that
such proceeds shall be deposited in our nominated Settlement Account upon receipt of the proceeds from the Issuer.
