I. Reflective log

It was happened when I was on duty during my clinical days in the medicine
ward. A female patient of 23 years old was admitted to medicine ward with
complain of upper abdominal pain, nausea , vomiting ,swollen belly and fluid
in the belly. Previously patient was also suffering from complain of pelvic
inflammatory disorder. So she was taking treatment for it but then her condition
was worse and she admitted to hospital. Her temperature was very high. She
also has complained of distended abdomen, low urine output with incontinemce,
burning mictuation, high grade fever. Then she was diagnosed for peritonitis.
For the management of this diagnose she was shifted to the HDU.

When I firstly encounter the client according to the situation I felt that she was
suffering from critical conditions because she has high grade fever, urine
incontinence, acute abdominal pain, highly increased CRP. Further I felt that it
is a hard time for the patient and her family because she was suffering from a
critical situation. But it was the time to not lose hope.

There were some positive and also negative aspects of the situation. Positive
aspect of the situation was the proper handling of patients by the health
professionals including staff and doctors. Negative aspect was the hopelessness
of the patient due to her disease condition.

After all this situation I analyzed that client need critical care to regain her
healthy life. According to the situation main aim was to monitor the BP. Next
step was the pain management for these purpose non-pharmacological
techniques were also applied. Fluid balance chart maintenance is very important
for the patient who is suffering from the peritonitis.IV fluids were administered
with great care. All the prescribed medicine were administered timely.

then I concluded that patient is suffering from life threatening condition so


critical care is very important for him at that condition. And after the care which
was done for her betterment become fruitful and patient condition has
improved.
My action plan for next time will depend on the situation to support.
