I, ABEL.NUDALO DALAGUIT Jr., of legal age, single, and a

resident of Barangay Pangdan, Naga City, Cebu, UNDER OATH do
hereby depose and state that:

1. THAT in my Certificate of Live Birth as recorded in the Office

of the Civil Registrar of __________, it is stated that I was born on
August 29, 1994;

2. THAT the information in the PNP records regarding my

date of birth was inadvertently inputted as August 28, 1994, thus
requiring clarification and correction of the erroneous data;

3. THAT I am executing this Affidavit to attest to the truth

of the foregoing facts, and to use the same for filing for a correction of
entry of the PNP records.
